Recent randomized controlled trials (RCTs) investigating the efficacy of HA-based sperm selection prior to ICSI, including HABSelect, have consistently reported reductions in the numbers of miscarriages among couples randomized to the intervention, suggesting a pathological sperm-mediated factor mitigated by prior HA-binding/selection. The mechanism of that protection is unknown.

Oocyte aging: looking beyond chromosome segregation errors

Summary: The age-associated decline in female fertility is mainly due to a decrease in oocyte quality. This decline is influenced by various maternal and environmental factors. An increase in the rate of meiotic errors is the main cause of the decline in oocyte developmental competence. However, as women age, abnormalities in the ooplasm, such as altered metabolism, organelle dysfunction, and aberrant gene regulation, gradually weaken oocyte quality. Accumulation of maternal macromolecules during folliculogenesis is crucial for oocyte competence, as transcription is not required for oocyte maturation, fertilization, and early embryo development. Factors such as assisted reproduction technologies (ARTs) or environmental changes can interfere with the crucial remodeling of oocyte epigenetics during oogenesis, and the effects may be amplified by reproductive aging. Since the molecular mechanisms of maternal aging on human oocyte function are not well understood, studies in animal models are necessary to deepen our understanding and have translational implications for human ARTs.

Among good quality blastocysts, there was no significant difference between day 5 and day 6 TE biopsy rates. However, for fair and poor quality blastocysts, day 5 TE biopsy had significantly higher rates than day 6 TE biopsy. Neither blastocyst grade nor TE biopsy day significantly affected the miscarriage rate. Neonatal outcomes such as newborn sex, gestational age, preterm birth, birth weight, and low birth weight did not significantly differ between day 5 and day 6 TE biopsies. Both blastocyst grade and TE biopsy day should be considered together in preimplantation genetic testing-frozen embryo transfer.
