Anti-Mullerian hormone as a marker of ovarian reserve and premature ovarian insufficiency in children and women with cancer: a systematic review

Summary: Female patients undergoing anticancer treatment are at risk of adverse ovarian outcomes, and AMH can be used as a biomarker to assess ovarian reserve and POI. Chemotherapy or radiotherapy consistently led to reduced/undetectable AMH levels, but partial recovery was observed during follow-up. Oligo/amenorrhea was associated with post-treatment AMH decline, suggesting impending POI. Long-term follow-up of pediatric patients showed significantly lower AMH levels compared to control groups. AMH can identify the damaging effect of cancer treatments on ovarian function, but further studies are needed to determine its diagnostic and predictive value.

Summary: The present findings suggest that AMH-mediated follicle atresia only occurs in early follicles before they become sensitive to FSH. Additionally, the study found that AMH overexpression leads to increased apoptosis rates of granulosa cells and oocytes in the ovaries. This indicates that the role of AMH is not to preserve ovarian reserve, but to prevent excessive follicle accumulation.
