Predicting the survival benefit of liver transplantation in HBV-related acute-on-chronic liver failure: an observational cohort study

Peng Li et al.

Summary: This study aimed to identify an appropriate score for predicting the survival benefit of liver transplantation (LT) in HBV-related acute-on-chronic liver failure (ACLF) patients. The results showed that the COSSH-ACLF IIs score accurately predicted the risk of death on the waitlist and post-LT mortality, and patients with a score of 7-10 derived a higher net survival benefit from LT.

Outcomes of living-donor liver transplantation for acute-on-chronic liver failure based on newly proposed criteria in Japan

Takeo Toshima et al.

Summary: The new diagnostic criteria for ACLF in Japan have shown feasibility in classifying patients undergoing liver transplantation. The presence and severity of ACLF, particularly the presence of multiple organ failures, lead to increased morbidity and mortality after transplantation.

Liver Transplantation in Acute-on-Chronic Liver Failure: Excellent Outcome and Difficult Posttransplant Course

Guang-Hou Chen et al.

Summary: This study compared the prognosis of transplanted ACLF patients to nontransplanted ACLF patients and decompensated cirrhosis recipients. Liver transplantation was found to greatly improve the survival of ACLF patients, although they had more burdens and higher complication morbidity compared to decompensated cirrhosis recipients.

Liver transplantation for acute-on-chronic liver failure predicts post-transplant mortality and impaired long-term quality of life

Lukas Goosmann et al.

Summary: Among cirrhosis patients, acute-on-chronic liver failure (ACLF) is associated with reduced post-liver transplantation (LT) survival mainly due to infections. Long-term follow-up revealed comparable graft function and comorbidity burden in ACLF-LT and non-ACLF-LT survivors, but many ACLF-LT recipients experienced significantly impaired health and quality of life.

'First week' is the crucial period for deciding living donor liver transplantation in patients with acute-on-chronic liver failure

Ashok Choudhury et al.

Summary: ACLF is a rapidly progressive disease, and timely risk stratification within the first week of hospitalization is crucial. Delay in liver transplant up to 7 days can lead to a higher incidence of multiorgan failure and increased mortality rates. Therefore, defining 'Emergent LT' in the first week is important for improving survival in ACLF patients.

Therapeutic plasma-exchange improves systemic inflammation and survival in acute-on-chronic liver failure: A propensity-score matched study from AARC

Rakhi Maiwall et al.

Summary: Plasma-exchange has been shown to improve systemic inflammation and lower the development of multiorgan failure in patients with acute liver failure. It provides significant survival benefit over Fractional Plasma Separation and Adsorption and could be a preferred modality of liver support for ACLF patients. Plasma-exchange cleared inflammatory cytokines, damage-associated molecular patterns, and endotoxin in all patients, and responders showed improved immune function and cytokine levels compared to non-responders.

Predictors and outcome of emergent Liver transplantation for patients with acute-on-chronic liver failure

Ji Eun Kim et al.

Summary: Emergent liver transplantation provided significant survival benefit to patients with acute-on-chronic liver failure, regardless of their baseline ACLF grade. Baseline MELD scores and ACLF progression during the pre-transplant period were associated with post-LT survival outcomes.

Excellent Outcome in Living Donor Liver Transplantation: Treating Patients With Acute-on-Chronic Liver Failure

Yu-Chen Wang et al.

Summary: The study investigated the outcomes of living donor liver transplantation in patients with acute-on-chronic liver failure (ACLF) and found excellent results in carefully selected patients, regardless of the number of organ failures. Survival rates and surgical outcomes for ACLF patients were comparable to non-ACLF patients, highlighting the importance of timely transplantation and comprehensive perioperative care in saving ACLF patients' lives.

Acute on Chronic Liver Failure: Factors Associated With Transplantation

Naeem Goussous et al.

Summary: Liver transplantation for appropriately selected patients with ACLF can lead to a significantly higher 1-year survival rate, with transplanted patients having fewer failed organs and a lower incidence of ACLF grade 3 compared to nontransplanted patients. Additionally, hospital charges were significantly higher for transplanted patients, indicating the financial burden of liver transplantation.

Plasma Exchange in Acute and Acute on Chronic Liver Failure

Rakhi Maiwall et al.

Summary: Therapeutic plasma exchange (TPE) has emerged as an attractive extracorporeal blood purification technique in patients with acute liver failure (ALF) and acute on chronic liver failure (ACLF), showing benefits by promoting liver regeneration through the removal of toxic substances. TPE may serve as a liver support device for patients with failing liver until liver transplantation or clinical recovery, with routine high-volume TPE recommended for ALF patients but less robust data for ACLF patients.
