Patient Identification for Serious Illness Conversations: A Scoping Review

Rebecca Baxter et al.

Summary: This study examined the current literature on patient identification for serious illness conversations and found that patients were primarily identified using clinical/diagnostic triggers, the 'surprise question', or a combination of methods. A diverse assortment of clinicians and non-clinical resources were involved in the identification process.

Validation of prognostic indices for short term mortality in an incident dialysis population of older adults >75

Bjorg Thorsteinsdottir et al.

Summary: The study validated six prognostic indices to predict 3 and/or 6 months mortality in elderly incident dialysis patients. Results showed that the Wick and Foley indices performed the best, but had issues with under and over calibration. More accurate indices are needed for predicting survival in older patients with kidney failure.

Summary: Elderly patients entering RRT commonly have comorbidities, but the risk of death associated with comorbidities is lower than in younger patients, and the increase in mortality rates associated with an increasing number of comorbidities is smaller.
