Development of a tool to detect small airways dysfunction in asthma clinical practice

Summary: This study aimed to develop a simple tool to predict small airways dysfunction (SAD) in asthma, using items from the Small Airways Dysfunction Tool (SADT) questionnaire, basic patient characteristics, and available respiratory tests. Logistic regression models were built using key SADT items and clinical information to predict the likelihood of SAD. Adding spirometry and oscillometry to the models increased the diagnostic ability.

Summary: The distribution of airway smooth muscle (ASM) remodeling in asthma varies widely within and between individuals, and is related to asthma severity. Despite the heterogeneous distribution, ASM remodeling is present in the majority of asthma cases, both in large and small airways. These findings emphasize the importance of patient-specific targeting of ASM remodeling in asthma treatment.

Small airways dysfunction is associated with increased exacerbations in patients with asthma

Summary: This study found that asthmatic patients with a history of exacerbations in the last year had worse peripheral airway function, while those recently treated for an asthma exacerbation had comparable peripheral airway function to the stable group. These findings suggest that active peripheral airway dysfunction may play a role in the pathophysiology of asthma exacerbations.

Results showed that heavy smokers had significantly higher values of IOS index of SAD compared to mild smokers and never-smokers, indicating the potential of IOS in detecting SAD in this population.
