[68 Ga]Ga-PSMA-11 Small Bowel Uptake in Crohn's Disease: Revisiting the Non-specificity of PSMA Ligands

Summary: The clinical application of prostate-specific membrane antigen (PSMA) PET/CT in prostate cancer has led to the discovery that other non-prostatic diseases can also exhibit PSMA radioligand uptake. We present a case of a 71-year-old male with prostate cancer who underwent PSMA PET/CT for recurrence evaluation. The scan revealed PSMA-11 uptake in the distal ileum, a finding which was later attributed to the patient's known Crohn's disease. This case highlights the importance of recognizing non-prostatic benign pathological PSMA uptake in patients with coexisting diseases, as it can be misinterpreted in the evaluation of prostate cancer.

Ga-68 PSMA-PET/CT showed higher specificity and accuracy in pre-operatively diagnosing primary pancreatic malignancy compared to FDG-PET/CT.
