68Ga-DOTATOC PET/MR imaging and radiomic parameters in predicting histopathological prognostic factors in patients with pancreatic neuroendocrine well-differentiated tumours

P. Mapelli et al.

Summary: This study explores the role of fully hybrid Ga-68-DOTATOC PET/MR imaging and radiomic parameters in predicting histopathological prognostic factors in patients with pancreatic neuroendocrine tumors (PanNETs) undergoing surgery. The study demonstrates that PET and MRI imaging parameters are significantly correlated with lymph node involvement and have good predictive performance.

Non-functional pancreatic neuroendocrine tumours: ATRX/DAXX and alternative lengthening of telomeres (ALT) are prognostically independent from ARX/PDX1 expression and tumour size

Wenzel M. Hackeng et al.

Summary: Recent studies have shown that ATRX/DAXX and ALT are important prognostic factors for NF-PanNETs, especially in tumors less than or equal to 2.0 cm. These biomarkers are highly specific for pancreatic origin among NET metastases of unknown primary.

A Guide to ComBat Harmonization of Imaging Biomarkers in Multicenter Studies

Fanny Orlhac et al.

Summary: The impact of PET image acquisition and reconstruction parameters on SUV measurements or radiomic feature values is discussed in this article. The use of the ComBat method to reduce the scanner effect is explained with practical examples. Situations where the ComBat assumptions are not met and should not be used are also presented.

Summary: In sporadic nonfunctioning pancreatic neuroendocrine tumors (NF-PanNETs), CE-CT, EUS, and Ga-68-DOTATOC PET have low sensitivity but high specificity in predicting lymph node metastases.

Molecular profile of pancreatic neuroendocrine neoplasms (PanNENs): Opportunities for personalized therapies

Jemma Arakelyan et al.

Summary: Pancreatic neuroendocrine neoplasms (panNENs) present challenges in predicting clinical behavior and response to therapies due to high heterogeneity, with most being nonfunctional and advanced at diagnosis. Somatostatin receptors are the only established prognostic and predictive biomarkers, while commonly mutated genes include MEN1 and chromatin remodeling genes. Differentiated neuroendocrine carcinomas (panNECs) have distinct mutations and poor prognosis. Ongoing epigenomic research offers opportunities to enhance understanding and develop personalized therapies for these tumors.

Dual Tracer 68Ga-DOTATOC and 18F-FDG PET Improve Preoperative Evaluation of Aggressiveness in Resectable Pancreatic Neuroendocrine Neoplasms

Paola Mapelli et al.

Summary: This study aimed to define an imaging risk profile for Pancreatic neuroendocrine neoplasms (PanNENs) candidates to surgery by assessing the predictive role of 68Ga-DOTATOC and 18F-FDG PET/CT and PET/MR derived parameters in risk stratification. The results showed that specific PET parameters such as 68Ga-DOTATOC SUVmax, 18F-FDG MTV, and TLG were able to predict tumour stage, metastases, and aggressiveness, providing valuable information for diagnostic preoperative work-up.

Prognostic Significance of Altered ATRX/DAXX Gene in Pancreatic Neuroendocrine Tumors: A Meta-Analysis

Fei Wang et al.

Summary: This study is the first rigorous pooled analysis assessing ATRX/DAXX mutations as prognostic biomarkers in PanNETs. The results indicate that patients with altered ATRX/DAXX genes have poor DFS, while altered ATRX/DAXX genes in metastatic patients show a trend towards improved overall survival.

Matteo Tacelli et al.

Summary: The study aimed to investigate the diagnostic accuracy of preoperative EUS-FNA Ki-67 evaluation on PanNENs and the impact of misclassification on prognosis. Results showed that approximately 25% of patients were misclassified, highlighting the importance of carefully considering misclassification impact, especially in small tumors undergoing observation.
