Inflammatory pathways in heart failure with preserved left ventricular ejection fraction: implications for future interventions

Nicola Riccardo Pugliese et al.

Summary: Heart failure with preserved ejection fraction (HFpEF) is a complex syndrome mainly affecting older people with many comorbidities. The management of HFpEF currently focuses on symptom control and treatment of comorbid conditions. Inflammation may play a key role in the development and progression of HFpEF.

Central and peripheral sympathetic activation in heart failure

Guido Grassi et al.

Summary: The overdrive of the sympathetic nervous system in heart failure has been reported for over half a century. Recent advancements in methodological approaches have provided a better understanding of various aspects of neuroadrenergic alterations, including regional differences, the role of the central nervous system, and the involvement of reflex mechanisms. Additionally, the impact of sympathetic activation on cardiovascular organs and potential therapeutic interventions have been highlighted.

Exercise-induced pulmonary hypertension in HFpEF and HFrEF: Different pathophysiologic mechanism behind similar functional impairment

Nicola Riccardo Pugliese et al.

Summary: Despite similar disease severity, HFpEF and HFrEF show different pathophysiological mechanisms. HFpEF is characterized by a worse LA-LV and RV-PA interaction than HFrEF, with more prevalent low-grade systemic inflammation. These features may have a role in exercise-induced pulmonary hypertension in HFpEF.

Ventricular-Arterial Coupling Derived From Proximal Aortic Stiffness and Aerobic Capacity Across the Heart Failure Spectrum

Nicola Riccardo Pugliese et al.

Summary: This study evaluated the ventricular-arterial coupling in heart failure patients and found that abnormal ventricular-arterial coupling is directly correlated with the severity of heart failure and worse functional capacity. Furthermore, heart failure patients with preserved ejection fraction have worse ventricular-arterial coupling.

Characterization of hemodynamic and metabolic abnormalities in the heart failure spectrum the role of combined cardiopulmonary and exercise echocardiography stress test

Nicola R. Pugliese et al.

Summary: Heart failure is a complex clinical syndrome characterized by different etiologies and a broad spectrum of cardiac structural and functional abnormalities. Current guidelines classify heart failure based on left ventricular ejection fraction, distinguishing between reduced and preserved ejection fraction. Heart failure can progress from asymptomatic stages to clinically manifest syndrome, and the progression is associated with prognosis. However, the understanding of the steps and risk factors leading to heart failure progression is still lacking, especially in preserved ejection fraction heart failure. Precise hemodynamic and metabolic characterization may help identify different disease trajectories and risk factors, leading to specific treatment targets.

Age- and gender-specific upper limits and reference equations for workload-indexed systolic blood pressure response during bicycle ergometry

Kristofer Hedman et al.

Summary: This study provides new reference values for the systolic blood pressure response to exercise workload, aiding in interpretation of SBP response and potentially guiding future research on the prognostic impact. Females show significantly higher SBP in relation to workload compared to males, suggesting greater peripheral vascular resistance during exercise.

Cardiac Reserve and Exercise Capacity: Insights from Combined Cardiopulmonary and Exercise Echocardiography Stress Testing

Nicola Riccardo Pugliese et al.

Summary: Exercise stress echocardiography can be used to assess VO2 levels in heart failure patients, with measures of LV systolic function, LA compliance, and right ventricle-pulmonary artery coupling being directly related to peak VO2, independently of other factors. This provides important insights into effort intolerance in HF patients.

Summary: Gender differences in blood pressure regulation during rest and exercise were observed in athletes, with male athletes showing lower blood pressure response per workload compared to female athletes. Vascular functional parameters were not able to predict the workload-indexed blood pressure response in males and females.

Mechanisms of reduced peak oxygen consumption in subjects with uncomplicated type 2 diabetes

Lorenzo Nesti et al.

Summary: This study aimed to quantify effort intolerance in T2D and dissect the associated cardiopulmonary alterations. The results showed that effort intolerance and reduced VO2peak are severe and highly prevalent conditions in uncomplicated T2D patients, resulting primarily from a major defect in skeletal muscle oxygen extraction and subtle myocardial systolic dysfunction.
