Nonadherence in Hypertension: How to Develop and Implement Chemical Adherence Testing

Dan Lane et al.

Summary: Nonadherence to antihypertensive medication is common, and chemical adherence testing is a reliable method to detect nonadherence, especially in patients with apparent treatment-resistant hypertension. Testing should be done in specific situations, such as in those with resistant hypertension or those on multiple antihypertensives with inadequate blood pressure control. Verbal consent should be obtained before testing, and results should be discussed with the patient for better understanding.

Catheter-based ultrasound renal denervation in patients with resistant hypertension: the randomized, controlled REQUIRE trial

Kazuomi Kario et al.

Summary: This study found no significant difference in ambulatory blood pressure reductions between renal denervation and a sham procedure in treated patients with resistant hypertension. Although the blood pressure reduction after renal denervation was similar to other sham-controlled studies, the unexpected greater reduction in the sham control group in this study highlights design issues that will be addressed in a new trial.

Renal denervation in patients with versus without chronic kidney disease: results from the Global SYMPLICITY Registry with follow-up data of 3 years

Christian Ott et al.

Summary: The blood pressure lowering effects of renal denervation (RDN) were compared between hypertensive patients with or without chronic kidney disease (CKD). The results showed that the reduction in 24-hour systolic and diastolic blood pressure was similar in both groups after RDN, but the reduction in office systolic blood pressure was less in patients with CKD.

Summary: In this study, the long-term safety and efficacy of renal denervation were evaluated. The results showed that renal denervation produced a clinically meaningful and lasting blood pressure reduction up to 36 months, independent of concomitant antihypertensive medications and without major safety events.

Summary: The experimental results on pigs undergoing RF RDN showed that immediate nerve changes included necrosis in the ablated region, which gradually resolved over time. The scoring for nerve fiber atrophy indicated significant axonal loss within and downstream to the ablation regions. Levels of NE and axon density in the renal cortex remained significantly reduced postoperatively.

Summary: The 36-month follow-up of the SYMPLICITY HTN-3 trial demonstrates the long-term safety and efficacy of renal artery denervation in patients with treatment-resistant hypertension. Patients who received renal artery denervation showed significant reductions in blood pressure and better blood pressure control compared to those who received sham control.

Weili Zhang et al.

Summary: In 8511 Chinese patients aged 60 to 80 with hypertension, intensive treatment with a systolic blood-pressure target of 110 to <130 mm Hg resulted in a lower incidence of cardiovascular events compared to standard treatment with a target of 130 to <150 mm Hg. Except for a higher incidence of hypotension, the two groups had similar rates of adverse events.
