ILAE definition of the Idiopathic Generalized Epilepsy Syndromes: Position statement by the ILAE Task Force on Nosology and Definitions

Edouard Hirsch et al.

Summary: This paper aims to define the four syndromes comprising the idiopathic generalized epilepsies (IGEs) and provides updated diagnostic criteria. For patients who do not meet the criteria for these syndromes but have generalized seizure types, a classification is also provided. Recognizing these syndromes as a special grouping helps determine prognosis and treatment implications.

ILAE classification and definition of epilepsy syndromes with onset in neonates and infants: Position statement by the ILAE Task Force on Nosology and Definitions

Sameer M. Zuberi et al.

Summary: This article proposes a classification and definition of epilepsy syndromes in neonates and infants, aiming to support epilepsy diagnosis and emphasize the importance of classifying epilepsy by syndrome and etiology. The article reports the epidemiology, clinical course, seizure types, EEG, neuroimaging, genetics, and differential diagnosis for each syndrome, and presents self-limited syndromes, developmental and epileptic encephalopathies, and etiology-specific epilepsy syndromes.

International League Against Epilepsy classification and definition of epilepsy syndromes with onset in childhood: Position paper by the ILAE Task Force on Nosology and Definitions

Nicola Specchio et al.

Summary: The 2017 International League Against Epilepsy classification defines a three-tier system for epilepsy syndromes, with childhood onset epilepsy syndromes falling into three categories: self-limited focal epilepsies, generalized epilepsies, and developmental and/or epileptic encephalopathies. Identification of specific syndromes helps guide management and prognosis. This paper describes these childhood onset epilepsy syndromes, highlighting their seizure types, EEG features, phenotypic variations, and key investigation findings.

2022 Overview of Metabolic Epilepsies

Birute Tumiene et al.

Summary: Understanding the genetic architecture of metabolic epilepsies is crucial for clinical practice and further research. This study aimed to identify the scope of metabolic epilepsies and investigate their clinical presentation, diagnostic approaches, and treatments. A total of 600 metabolic epilepsies were identified, many of which are complex and multisystem disorders. Abnormalities in routine laboratory tests and/or metabolic testing can be found in 70% of metabolic epilepsies, though often nonspecific. Specific treatments exist for 18% of metabolic epilepsies, which can significantly improve health outcomes if diagnosed in time.

The ILAE consensus classification of focal cortical dysplasia: An update proposed by an ad hoc task force of the ILAE diagnostic methods commission

Imad Najm et al.

Summary: Ongoing challenges in diagnosing focal cortical dysplasia (FCD) require continuous research and consensus agreement to improve disease definition and classification. An International League Against Epilepsy (ILAE) Task Force has reviewed the FCD classification of 2011 and identified existing gaps. They conducted surveys, literature reviews, and clinico-pathological studies to update the classification and propose new categories. This update may contribute to a better understanding of FCD and the development of targeted treatments.

Autoantibody Encephalitis: Presentation, Diagnosis, and Management

Eric Lancaster

Summary: Autoantibody encephalitis is a disease that causes clinical syndromes characterized by altered mentation, abnormal movements, seizures, psychiatric symptoms, etc. The diagnosis can be confirmed through antibody tests, which may be better detected in spinal fluid or serum. Understanding the pharmacology of receptors can provide insights into the syndromes, and immune therapy is generally effective in treating these disorders.

Frequent SLC35A2 brain mosaicism in mild malformation of cortical development with oligodendroglial hyperplasia in epilepsy (MOGHE)

Thomas Bonduelle et al.

Summary: Focal malformations of cortical development are associated with somatic brain mutations during neurodevelopment, while MOGHE is a newly recognized entity linked to pediatric drug-resistant focal epilepsy, amenable to neurosurgical treatment. The study identified somatic pathogenic SLC35A2 variants in MOGHE patients, suggesting a genetic marker for this condition.

Summary: The meta-analysis aimed to evaluate the clinical use value of C-11-FMZ and F-18-FDG in PET for localizing the epileptogenic zone. The pooled concordance rates of FDG-PET, FMZ-PET, and FDG-PET/MRI were calculated to be 0.67, 0.75, and 0.93, respectively, showing that FDG-PET/MRI had the highest concordance rate. The overall sensitivity and specificity of F-18-FDG-PET were 0.66 and 0.71, while C-11-FMZ-PET had a sensitivity of 0.62 and specificity of 0.73.

Seizure Semiology, EEG, and Imaging Findings in Epilepsy Secondary to Mitochondrial Disease

Anthony L. Fine et al.

Summary: In this retrospective case series, 20 patients with epilepsy and genetically confirmed mitochondrial disorders were studied. The study revealed a higher incidence of seizures originating from the posterior cerebral regions, with EEG and imaging abnormalities increasingly seen in those regions over time. Focal seizures and focal status epilepticus with visual symptoms were common presentations. Nutraceutical supplementation showed no significant impact on disease progression in these patients.

Review of treatment and therapeutic targets in brain arteriovenous malformation

Peipei Pan et al.

Summary: Brain arteriovenous malformations (bAVM) are a significant cause of intracranial hemorrhage, especially in younger patients. The pathogenesis of bAVM remains largely unknown, but recent discoveries have identified potential therapeutic targets through studying genetic syndromes, animal models, and surgically resected specimens. Studies are currently underway to investigate new treatment options and potential therapeutic targets uncovered by these discoveries.
