Summary: This study investigated the incidence and disease course of perianal Crohn's disease in adult patients over a 19-year period, finding a stable incidence rate over time. Patients with perianal CD were at increased risk of undergoing major surgery and had a higher risk of developing anal and rectal cancer compared to non-IBD matched controls.

Managing Complex Perianal Fistulizing Disease

Karina E. Pedersen et al.

Summary: Perianal disease is a severe phenotype of Crohn's disease that affects up to one third of patients and significantly reduces quality of life. Conventional medical therapy and surgery have limited efficacy, with only a third of patients achieving long-term durable remission. Emerging therapies such as mesenchymal stem cell therapy and laser therapy have shown improved efficacy compared to traditional treatments, but up to 40% of patients may still require a stoma related to perianal fistulizing disease.
