Challenges with Cell-based Therapies for Type 1 Diabetes Mellitus

Prakriti Siwakoti et al.

Summary: Type 1 diabetes is a chronic metabolic disease characterized by the loss of insulin-producing beta cells. Current treatment involves lifelong dependence on insulin injections. Cell-based therapies and bioengineered devices are being investigated as potential cures, but face challenges such as limited tissue availability and high costs.

Adult-onset type 1 diabetes: A changing perspective

J. Burahmah et al.

Summary: Type 1 diabetes can occur in adults and may not require insulin therapy initially. This review compares adult-onset and childhood-onset type 1 diabetes, discusses their epidemiology, genetics, diagnostic criteria, and management, and also looks at the psychosocial challenges faced by T1D patients and how to manage them.

Mesenchymal stem cell transplantation in newly diagnosed type-1 diabetes patients: a phase I/II randomized placebo-controlled clinical trial

Mahmoud Izadi et al.

Summary: This study evaluated the safety and efficacy of autologous bone marrow-derived mesenchymal stem cells (MSCs) transplantation via intravenous injection in newly diagnosed type-1 diabetes (T1D) patients. The results showed that MSCs transplantation is safe, reduces hypoglycemic episodes, improves glycemic control and immune function, and enhances quality of life in T1D patients.

Safety, immunological effects and clinical response in a phase I trial of umbilical cord mesenchymal stromal cells in patients with treatment refractory SLE

Diane L. Kamen et al.

Summary: In this exploratory phase I trial, umbilical cord derived mesenchymal stromal cells (MSCs) were used to treat refractory lupus patients. The results showed that the treatment was safe and effective, with significant reductions in specific cells and autoantibody levels. The study provides novel insights into the mechanisms by which MSCs impact disease.

Global incidence, prevalence, and mortality of type 1 diabetes in 2021 with projection to 2040: a modelling study br

Gabriel A. Gregory et al.

Summary: This study developed a model based on available data to estimate the prevalence, incidence, associated mortality, and life expectancy of type 1 diabetes for 201 countries in 2021. The study also projected the prevalent cases in 2040. The burden of type 1 diabetes was found to be significant and expected to increase rapidly, particularly in resource-limited countries.

Residual beta cell function in long-term type 1 diabetes associates with reduced incidence of hypoglycemia

Rose A. Gubitosi-Klug et al.

Summary: The study found that long-term type 1 diabetes mellitus patients may still have residual beta cell function, which is associated with a reduced prevalence of severe hypoglycemia.

One repeated transplantation of allogeneic umbilical cord mesenchymal stromal cells in type 1 diabetes: an open parallel controlled clinical study

Jing Lu et al.

Summary: The study demonstrated that one repeated intravenous dose of allogeneic UC-MSCs is safe in people with recent-onset T1D and may result in better islet beta cell preservation during the first year after diagnosis compared to standard treatment alone. The MSC-treated group showed a significantly higher rate of achieving clinical remission compared to the control arm, with some individuals even achieving insulin independence. Multivariable logistic regression analysis showed that lower fasting C-peptide and higher dose of UC-MSC correlated with the achievement of clinical remission after transplantation, with no severe side effects observed.

Adipose tissue-derived stromal/stem cells plus cholecalciferol: a pilot study in recent-onset type 1 diabetes patients

Joana Rodrigues Dantas et al.

Summary: This study investigated the safety and efficacy of ASCs and vitamin D in patients with recent-onset T1D. The results showed that allogenic ASCs + VIT D treatment for 6 months increased basal CP, reduced insulin requirements, and HbA1c levels, and may have a role in preserving beta-cells in patients with recent-onset T1D.
