Inequity in Adoption of Advanced Diabetes Technologies Among Medicare Fee-for-service Beneficiaries

Kael Wherry et al.

Summary: There are significant racial and ethnic differences in the management of type 1 diabetes (T1D), particularly in the adoption of diabetic technology. Between 2017 and 2019, the gap in insulin pump use between Black and White beneficiaries increased. Black patients were more likely to be eligible for Medicare/Medicaid and have disabilities or end-stage renal disease. These findings highlight the importance of addressing health inequities in T1D management.

Continuous glucose monitoring in primary care - are we there?

Thomas W. Martens

Summary: This review examines the expanding role of continuous glucose monitoring in glycemic management in primary care, highlighting the convergence of several factors as crucial for optimized use of this technology. While significant progress has been made in recent years in all three areas, there is still work to be done in creating systems of support for widespread use of CGM in primary care.

Summary: This study assessed the clinical effects of using real-time continuous glucose monitoring (rtCGM) in patients with type 2 diabetes (T2D) treated with basal insulin only or noninsulin therapy. The results showed that rtCGM use was associated with significant glycemic improvements. Further research is needed to modify insurance eligibility criteria to expand rtCGM use.

Making sense of glucose metrics in diabetes: linkage between postprandial glucose (PPG), time in range (TIR) & hemoglobin A1c (A1C)

Thomas W. Martens et al.

Summary: In addition to A1C, CGM technology provides richer glucose data to help diabetes patients manage their disease more safely and effectively. CGM software offers valuable data and facilitates goal setting and self-management discussions between clinicians and patients. By focusing on factors such as time in range, time below range, and postprandial glucose-induced time above range, the safety and efficacy of glucose management can be improved.

Summary: Continuous glucose monitoring (CGM) has been proven to improve glycemia control and quality of life for individuals with type 1 and type 2 diabetes, but many primary care physicians are unfamiliar with interpreting and utilizing CGM data. As the adoption of this technology continues to grow, physicians will be challenged to integrate CGM into their clinical practices effectively.

Integrating Continuous Glucose Monitoring Into Clinical Practices and Patients' Lives

Irl B. Hirsch et al.

Summary: Numerous studies have shown the benefits of continuous glucose monitoring (CGM) for individuals with diabetes. While demand for CGM in primary care is increasing, many clinicians are hesitant to prescribe it due to lack of familiarity with different devices and uncertainty about which device is best for each patient. This article reviews recent commercially available CGM devices and provides guidance on integrating CGM into clinical practices.

Racial and Ethnic Disparities in Rates of Continuous Glucose Monitor Initiation and Continued Use in Children With Type 1 Diabetes

Summary: Racial/ethnic disparities exist in continuous glucose monitor (CGM) use among children with type 1 diabetes. The study found that lower CGM use in non-Hispanic Black children was due to lower rates of device initiation and higher rates of discontinuation. Interventions are needed to address both of these barriers in order to reduce disparities in CGM use.
