Persistence of C-reactive protein increased levels and high disease activity are predictors of cardiovascular disease in patients with axial spondyloarthritis

Summary: This study aimed to investigate if the persistence of high C-reactive protein (CRP) levels and high disease activity can be predictive factors of cardiovascular disease (CVD) in patients with Axial Spondyloarthritis (axSpA). The results showed a strong association between the persistence of increased CRP levels, disease activity, and the occurrence of CV events in axSpA patients. These findings suggest that integrating these variables into an axSpA-specific CV risk score is necessary.

Endothelial dysfunction and increased carotid intima-media thickness in patients with spondyloarthritis without traditional cardiovascular risk factors

Summary: This study evaluated subclinical atherosclerosis in spondyloarthritis patients using three ultrasound methods and identified predictive factors for these parameters. The results showed that patients with spondyloarthritis had higher carotid intima-media thickness, lower flow-mediated dilation, and higher ankle brachial index compared to the control group. Carotid intima-media thickness was positively correlated with patient-related and disease parameters, while flow-mediated dilation was negatively correlated with male gender, age, blood pressure, creatinine, blood glucose, and Left Lequesne Index. Ankle brachial index was significantly associated with diastolic blood pressure. Multiple regression analysis identified age, total cholesterol, and creatinine as independent predictive factors for increased carotid intima-media thickness, and blood glucose and Left Lequesne Index as independent predictive factors for decreased flow-mediated dilation.

Summary: Recent studies have questioned the safety of using acetaminophen for pain relief and fever reduction in patients with cardiovascular disease and those taking anticoagulants. The studies suggest that acetaminophen, like most nonsteroidal anti-inflammatory drugs, can increase blood pressure and formulations containing sodium can increase cardiovascular risk. The influence of acetaminophen on coagulation through its effects on vitamin K metabolism is also discussed.
