Wellens syndrome An important consideration in patients with chest pain

Joshua Alexander et al.

Summary: Coronary artery disease (CAD) is the leading cause of death in the United States, with heart attacks occurring every 40 seconds. Acute coronary syndrome (ACS) is a potentially fatal complication of chronic CAD. Wellens syndrome, a rare form of ACS, is characterized by specific T-wave patterns on ECG and indicates critical left anterior descending coronary artery occlusion. Lack of familiarity with the ECG findings may lead to underrecognition of this syndrome by clinicians. This article discusses the significance of the Wellens ECG pattern in patients with chest pain.

Time for a new paradigm shift in myocardial infarction

Emre K. Aslanger et al.

Summary: The current STEMI/non-STEMI paradigm has limitations, missing a substantial proportion of ACO cases and leading to unnecessary catheterization lab activations. The evidence base for STEMI criteria is poor, not specifically designed to distinguish patients who would benefit from emergency reperfusion.

2021 AHA/ACC/ASE/CHEST/SAEM/SCCT/SCMR Guideline for the Evaluation and Diagnosis of Chest Pain

Martha Gulati et al.

Summary: This guideline provides recommendations and algorithms for clinicians to assess and diagnose chest pain in adult patients, based on contemporary evidence of risk stratification and diagnostic workup. Cost-value considerations in diagnostic testing have been incorporated, and shared decision-making with patients is recommended.

Accuracy of OMI ECG findings versus STEMI criteria for diagnosis of acute coronary occlusion myocardial infarction

H. Pendell Meyers et al.

Summary: This study compared the diagnostic accuracy of OMI ECG findings vs formal STEMI criteria in the diagnosis of acute myocardial infarction. Blinded interpretation of OMI ECG findings was found to be more accurate than using STEMI criteria, supporting further investigation into the OMI vs. NOMI paradigm.
