Endoscopic surveillance with systematic random biopsy for the early diagnosis of hereditary diffuse gastric cancer: a prospective 16-year longitudinal cohort study

Colin Y. C. Lee et al.

Summary: Hereditary diffuse gastric cancer, caused by CDH1 gene mutations, is characterized by early-onset signet ring cell carcinoma. Prophylactic total gastrectomy is the recommended treatment. This study assessed different sampling strategies for detecting signet ring cell carcinoma and identified criteria for characterizing endoscopic lesions in hereditary diffuse gastric cancer.

Confocal laser endomicroscopy in gastro-intestinal endoscopy: technical aspects and clinical applications

Nastazja Dagny Pilonis et al.

Summary: Confocal laser endomicroscopy (CLE) is an advanced endoscopic imaging technology that provides a magnified, cellular level view of gastrointestinal epithelia. It has moderate to high diagnostic accuracy for neoplastic and inflammatory conditions through the gastrointestinal tract. CLE can resolve diagnostic dilemmas in cases where conventional imaging fails and reduce the need for extensive tissue sampling. However, limitations include a narrow field of view, high capital costs, and specialized operator training requirements.

Summary: In surveillance for HDGC, the double-bite technique was significantly faster than the single-bite technique, but both groups had similar diagnostic yield for SRCC and biopsy quality.

Hereditary Diffuse Gastric Cancer: Approaches to Screening, Surveillance, and Treatment

Nastazja Dagny Pilonis et al.

Summary: Hereditary diffuse gastric cancer (HDGC) is a rare cancer syndrome associated with significant risks of gastric and lobular breast cancer, often presenting late in clinical settings. While most cases can be traced back to germline variants in the CDH1 gene, some families with clustering of DGC do not have a clear genetic cause. This review focuses on discussing key elements for assessing inherited DGC susceptibility, providing practical recommendations for surveillance and treatment of at-risk individuals and early-stage patients.

Evaluation of confocal laser endomicroscopy for detection of occult gastric carcinoma in CDH1 variant carriers

Samuel A. Schueler et al.

Summary: Confocal endomicroscopy alone has low sensitivity for occult cancer detection in CDH1 variant carriers, although it appeared no worse than the current recommended method and required fewer biopsies per patient. A more reliable endoscopic surveillance is needed as a viable alternative to surgery in this high-risk population.
