Predictors of disease recurrence after curative surgery for nonfunctioning pancreatic neuroendocrine neoplasms (NF-PanNENs): a systematic review and meta-analysis

V Andreasi et al.

Summary: This meta-analysis aimed to estimate the rate of disease recurrence and investigate risk factors for disease relapse in patients undergoing curative surgery for NF-PanNENs. The results showed a recurrence rate of approximately one out of five patients, with nodal involvement, tumor grade, microvascular, and perineural invasion identified as significant prognostic factors.

Multi-institutional Development and External Validation of a Nomogram to Predict Recurrence After Curative Resection of Pancreatic Neuroendocrine Tumors

Alessandra Pulvirenti et al.

Summary: The developed nomogram accurately predicts the probability of recurrence-free survival at 5 years after curative resection for PanNETs, showing improved accuracy over current staging systems. Individual recurrence risk estimation can assist in developing personalized surveillance programs post-surgery.

Outcomes after distal pancreatectomy for neuroendocrine neoplasms: a retrospective comparison between minimally invasive and open approach using propensity score weighting

Stefano Partelli et al.

Summary: The study found that compared to ODP surgery, MIDP surgery had lower postoperative complications and shorter length of stay, but fewer lymph nodes examined. There was no significant difference in disease-free survival and overall survival between the two groups.

Systematic Review and Metaanalysis of Lymph Node Metastases of Resected Pancreatic Neuroendocrine Tumors

Masayuki Tanaka et al.

Summary: This study suggests that lymph node metastasis is not uncommon in small and well-differentiated PNETs, and it is associated with worse prognosis. Reevaluation of the watch-and-wait strategy for small NF-PNETs is recommended, with consideration of oncologic resection with lymphadenectomy. More prospective and controlled studies are needed in the future.

Prognostic Role of Examined and Positive Lymph Nodes after Distal Pancreatectomy for Non-Functioning Neuroendocrine Neoplasms

Giovanni Guarneri et al.

Summary: A minimal number of 12 nodes should be harvested in case of DP for NF-PanNET for an appropriate nodal staging. The number of positive lymph nodes is an independent predictor of DFS after DP for NF-PanNET, and the N0/N1/N2 nodal classification seems to be more relevant than the current N0/N+ staging.

c-Myc promotes lymphatic metastasis of pancreatic neuroendocrine tumor through VEGFC upregulation

Tsung-Ming Chang et al.

Summary: Pancreatic neuroendocrine tumor (pNET) is a type of pancreatic tumor with neuroendocrine differentiation. Early stage pNET can be treated with surgical resection for long-term survival, while pNET with metastasis has a poor prognosis. In this study, it was found that c-Myc overexpression is associated with lymph node metastasis in pNET xenograft mice. Combination therapy using an mTOR inhibitor and c-Myc inhibitor or VEGFC-neutralizing protein reduced lymph node metastasis in mice with c-Myc overexpression, indicating potential therapeutic targets for pNET.

Meta-Analysis of Prognostic Factors for Recurrence of Resected Well-Differentiated Pancreatic Neuroendocrine Tumors

Yuanliang Li et al.

Summary: This study included 10 articles on prognostic factors for WDPNETs recurrence, and found that G2 grade, positive lymph nodes and surgical resection margin, vascular invasion, and perineural invasion may affect disease-free survival of WDPNETs, suggesting the need for closer postoperative follow-up and potential benefit from adjuvant therapy.

The Impact of 68Gallium DOTA PET/CT in Managing Patients With Sporadic and Familial Pancreatic Neuroendocrine Tumours

Daniel J. Cuthbertson et al.

Summary: This study evaluated the role of (68)Gallium (Ga-68)-DOTA PET/CT in managing patients with panNETs, showing that the imaging findings provided additional information in 54% of patients and influenced management in 39% of cases. The PET/CT scan more accurately stages and guides treatment in patients with sporadic/familial panNETs.

Sporadic non-functioning pancreatic neuroendocrine tumours: multicentre analysis

C. Ricci et al.

Summary: This study evaluated outcomes after surgery for sporadic pancreatic neuroendocrine neoplasms (Pan-NENs), identifying 2019 WHO grade and ENETS TNM stage as important factors. Surgical approach may be without benefit for Pan-NECs, unnecessary for small G1 sporadic Pan-NENs, and insufficient for stage III-IV and G3 Pan-NENs.
