Summary: Unconfirmed penicillin allergy labels have negative impact on personal and public health, leading to the emergence and transmission of resistant bacteria. Efforts have been made to promote proactive penicillin allergy evaluations and delabeling to remove inaccurate allergy labels. Access to penicillin allergy evaluation services is limited, but recent studies show the feasibility of expanding these services to a wider range of healthcare professionals.

Summary: Implementation of an antibiotic cross-reactivity chart combined with enhanced allergy assessment processes significantly improved the prescribing of beta-lactam antibiotics for surgical prophylaxis. The intervention group had a lower readmission rate and a lower percentage of patients receiving beta-lactam alternative antibiotics, but no significant difference in surgical site infections was observed.

Summary: This study showed that hypersensitivity reactions are uncommon in patients labeled as penicillin allergic. The frequency of immediate hypersensitivity reactions did not differ significantly among patients receiving cefazolin, clindamycin, or vancomycin. Avoiding cefazolin in patients labeled as penicillin allergic may not be necessary.
