Fragmentation of Different Calcification Growth Patterns in Bicuspid Valves During Balloon Valvuloplasty Procedure

Adi Morany et al.

Summary: This study investigates the calcification development and routes of type-1 bicuspid aortic valves based on CT scans and analyzes the impact of the unique geometrical shapes of calcium deposits on their fragmentation under balloon valvuloplasty procedures. It introduces a reverse calcification technique to predict calcification progression and identifies two main calcification patterns. Additionally, a calcification fragmentation biomechanical model is proposed to study the effect of balloon valvuloplasty on different calcification patterns.

A computational framework for post-TAVR cardiac conduction abnormality (CCA) risk assessment in patient-specific anatomy

Symon Reza et al.

Summary: This study suggests that elevated stresses in the membranous septum (MS) region generated during transcatheter aortic valve replacement (TAVR) may increase the risk of cardiac conduction abnormalities (CCA). The area-weighted average maximum principal logarithmic strain (AMPLS) in the MS region was found to be a strong predictive factor of CCA risk.

Assessment of Paravalvular Leak Severity and Thrombogenic Potential in Transcatheter Bicuspid Aortic Valve Replacements Using Patient-Specific Computational Modeling

Salwa B. Anam et al.

Summary: This study utilized patient-specific computational modeling to assess post-TAVR complications in BAV patients, finding a significant decrease in post-implantation paravalvular leak and thrombogenic potential with the latest generation TAVR devices. This highlights the potential of simulations in pre-procedural planning for better clinical outcomes and device selection.

The impact of hypo-attenuated leaflet thickening on haemodynamic valve deterioration following transcatheter aortic valve replacement

Summary: This study found that HALT is an independent predictor of haemodynamic valve deterioration (HVD), and specific depth and area thresholds were identified to predict HVD. CT following TAVR can determine patients at risk of HVD.

Treatment of Bicuspid Aortic Valve Stenosis with TAVR: Filling Knowledge Gaps Towards Reducing Complications

Breandan B. Yeats et al.

Summary: The purpose of this review is to study the treatment methods and influencing factors of bicuspid aortic valve (BAV) disease. Existing studies have found that BAV patients may have lower success rate and higher complication rate when undergoing transcatheter aortic valve replacement (TAVR) compared to tricuspid aortic valves. The asymmetrical morphology of BAV may also lead to asymmetric stent shapes, increasing the risk of thrombosis and decreasing durability. Computational modeling can help fill the gaps in understanding these complications and provide more accurate patient selection for TAVR.

Deformation of Transcatheter Aortic Valve Prostheses: Implications for Hypoattenuating Leaflet Thickening and Clinical Outcomes

Miho Fukui et al.

Summary: This study found that frame deformation, asymmetric leaflet, and smaller neosinus volume are associated with the occurrence of HALT in patients undergoing TAVR. The presence of HALT is associated with an increased risk of mortality within 1 year.

Transcatheter aortic valve replacement for bicuspid aortic valve disease: does conventional surgery have a future?

Breandan B. Yeats et al.

Summary: Bicuspid aortic valve (BAV) disease is the most common congenital heart valve defect, often associated with other heart conditions. Surgical aortic valve replacement (SAVR) is still the main treatment for severe cases, although transcatheter aortic valve replacement (TAVR) has emerged as an alternative. However, TAVR has shown higher rates of adverse outcomes in patients with significant calcification. In low surgical risk patients, SAVR still has advantages over TAVR. Additionally, BAV patients often have aortopathy, which can be treated with surgical techniques.

The Impact of Aortic Valvular Calcium on Transcatheter Heart Valve Distortion

Akihiro Nakajima et al.

Summary: The study revealed that eccentric calcification of the aortic valve is associated with longitudinal THV distortion. However, THV distortion does not impact paravalvular leak, aortic valvular mean pressure gradient, and adverse clinical events during midterm follow-up.

Outcomes 2 Years After Transcatheter Aortic Valve Replacement in Patients at Low Surgical Risk

Martin B. Leon et al.

Summary: In low surgical risk patients with symptomatic severe aortic stenosis, the PARTNER 3 trial showed superiority of transcatheter aortic valve replacement (TAVR) over surgery for the primary endpoint at 1 year. However, at 2 years, differences in death and stroke between TAVR and surgery were no longer significant, and TAVR patients had an increased risk of valve thrombosis.
