Massive transfusion and severe blood shortages: establishing and implementing predictors of futility

Domagoj Mladinov et al.

Summary: Massive transfusion protocols were designed for life-threatening hemorrhage, but determining when they are futile remains a challenge. Studies have not identified variables that can accurately predict early mortality, highlighting the importance of equitable distribution of limited blood resources. In this editorial, recent data and key aspects of developing tools to determine futility in massive transfusion are discussed.

of Transfusion Volume and Transfusion Rate as Markers of Futility During Ultramassive Blood Transfusion in Trauma

Ander Dorken Gallastegi et al.

Summary: Using a large national database, this study evaluated the relationship between RBC transfusion volume, RBC transfusion rate, and in-hospital mortality in trauma patients receiving ultramassive blood transfusion. The study found that RBC transfusion volume within 4 or 24 hours was not a marker of futility, but mean RBC transfusion rate within 4 hours had higher discriminatory ability and could inform resuscitation efforts in trauma patients receiving ongoing transfusion between 4 and 24 hours.

Outcomes after ultramassive transfusion in the modern era: An Eastern Association for the Surgery of Trauma multicenter study

Zachary A. Matthay et al.

Summary: Despite modern massive transfusion protocols, some trauma patients receiving ultramassive transfusion experience unbalanced ratios of red blood cells to other blood components, leading to increased mortality rates. Maintaining balanced ratios during ultramassive transfusion and considering certain clinical factors can aid in prognostication for these patients.
