Summary: De novo donor-specific human leucocyte antigen (HLA) antibodies are associated with increased risk of rejection and mortality in solid organ transplantation. Pre-transplant immunological risk stratification is currently based on the mean fluorescence intensity (MFI) of circulating donor-directed HLA antibodies. It is hypothesized that patients with higher epitope mismatch loads are at an increased risk of developing dnDSA. The use of HLA epitope-based matching tools could be beneficial for cardiothoracic transplant risk stratification and early intervention.
