Volume measurements on weightbearing computed tomography can detect subtle syndesmotic instability

Summary: This study aimed to determine the sensitivity and specificity of 2D area and 3D volume WBCT measurements for detecting subtle syndesmotic instability, as well as to evaluate changes in the 3D shape of the syndesmosis attributed to ligament injury types. Volumetric measurement up to 5 cm above the tibial plafond showed the highest sensitivity and specificity for diagnosing syndesmotic instability, especially in subtle cases. However, no specific pattern of 3D shape changes could be linked to a type of ligament rupture, indicating the need for further research and improvements in measurement tools.
