Impaired hydration status in acutely admitted older patients: prevalence and impact on mortality

Gianfranco Sanson et al.

Summary: Impaired hydration is common in older adults admitted acutely to medical care, and is associated with poor outcomes. Calculated serum osmolarity is an important tool for assessing dehydration and hypoosmolar disorders. Factors such as current dehydration, functional dependence, clinical instability, and high nutritional risk are associated with reduced short- and long-term survival.

Healthcare professionals' experiences with highly qualified nurses working in acute care teams in primary healthcare settings

Stine Emilie Junker Udesen et al.

Summary: This study explored healthcare professionals' experiences with a new Danish initiative aimed at strengthening primary healthcare in acute care units or teams. Results showed that participants were generally satisfied with the new acute care team and cross-sectorial possibilities, which increased the ability to assess and treat patients in their homes and potentially prevent unnecessary hospitalizations.

Municipal acute care teams as a flexible solution for the treatment of acutely ill patients at-home: a mixed-method study of patients' and caregivers' experiences with an acute care team

Stine Emilie Junker Udesen et al.

Summary: The study in Denmark showed that municipal acute care teams can effectively prevent hospitalisations and reduce caregivers' burden by providing at-home treatment. Patients and caregivers preferred at-home treatment in future similar situations, as it allowed them to maintain their lives. This initiative has been continued to focus on preventing hospitalisations.

Characteristics of outpatient emergency department visits of nursing home residents: an analysis of discharge letters

Stephanie Heinold et al.

Summary: Unplanned emergency department visits by nursing home residents are common, but many do not result in hospitalization. Research on diagnostic and therapeutic interventions during these visits is limited. Better coordination with general practitioners and improved training for nursing staff could potentially reduce unnecessary emergency department visits.
