Summary: Resistance to pyrrolobenzodiazepine (PBD)-conjugated antibody-drug conjugates (ADCs) is associated with downregulation of Schlafen family member 11 (SLFN11), a putative DNA/RNA helicase. Treatment with EZH2 inhibitor derepressed SLFN11 expression and increased sensitivity to PBD and PBD-conjugated ADCs. Combination of ATR inhibitor with PBD-based ADCs led to synergistic cytotoxicity in resistant cells or cells with SLFN11 knocked down via siRNA. This study provides insights into resistance mechanisms and potential strategies to overcome resistance to PBDs and PBD-conjugated ADCs.
