Summary: A decline in the number of applicants to cardiothoracic and vascular surgery (CTVS) training programs in recent years poses a challenge to India's healthcare system as the incidence of cardiovascular diseases continues to rise. The main reasons for students not choosing CTVS were a lack of required hospital infrastructure and a dependence on a multidisciplinary team. Students interested in CTVS mentioned the dynamic nature of the specialty and the excitement of being a cardiac surgeon as important motivators. Reforms in surgical training, such as mandatory rotations to CTVS, increased interactions with CTVS faculty, and incorporating catheter-based training in the CTVS curriculum, are urgently needed to attract more applicants to CTVS.
