Endoscopic submucosal dissection for superficial gastrointestinal lesions: European Society of Gastrointestinal Endoscopy (ESGE) Guideline - Update 2022

Pedro Pimentel-Nunes et al.

Summary: This guideline provides recommendations for the evaluation and treatment of superficial gastrointestinal lesions. It suggests that experienced endoscopists should use high definition white-light and chromoendoscopy for evaluation and recommends endoscopic submucosal dissection for most lesions. Different treatment approaches are suggested for specific situations. The guideline also provides recommendations for post-resection surveillance and treatment.

Short-Term Outcomes of Laparoscopic Proximal Gastrectomy With Double-Tract Reconstruction Versus Laparoscopic Total Gastrectomy for Upper Early Gastric Cancer: A KLASS 05 Randomized Clinical Trial

Sun-Hwi Hwang et al.

Summary: This study compared the short-term outcomes of laparoscopic proximal gastrectomy with double-tract reconstruction (LPG-DTR) and laparoscopic total gastrectomy (LTG) for upper early gastric cancer (EGC). The results showed no significant differences in operative time, estimated blood loss, morbidity rate, postoperative symptoms, and laboratory findings between the two groups.

Adjuvant chemotherapy is superior to chemoradiation after D2 surgery for gastric cancer in the per-protocol analysis of the randomized CRITICS trial

W. O. de Steur et al.

Summary: The per-protocol analysis of the CRITICS trial showed that patients who started the allocated post-operative treatment had a significantly better 5-year overall survival in the CT group compared to the CRT group. Patient and tumor characteristics were not different between the groups before the start of post-operative treatment.

Global cancer statistics 2020: GLOBOCAN estimates of incidence and mortality worldwide for 36 cancers in 185 countries

Hyuna Sung et al.

Summary: The global cancer burden in 2020 saw an estimated 19.3 million new cancer cases and almost 10.0 million cancer deaths. Female breast cancer surpassed lung cancer as the most commonly diagnosed cancer, while lung cancer remained the leading cause of cancer death. These trends are expected to rise in 2040, with transitioning countries experiencing a larger increase compared to transitioned countries due to demographic changes and risk factors associated with globalization and a growing economy. Efforts to improve cancer prevention measures and provision of cancer care in transitioning countries will be crucial for global cancer control.

D2 dissection improves disease-specific survival in advanced gastric cancer patients: 15-year follow-up results of the Italian Gastric Cancer Study Group D1 versus D2 randomised controlled trial

M. Degiuli et al.

Summary: This study compared the effects of D1 and D2 gastrectomy on gastric cancer patients, showing that D2 surgery can significantly improve survival and reduce gastric cancer-related mortality in patients with advanced disease and lymph node metastasis. D1 surgery may be more effective in older patients and early-stage disease.

Is splenic hilar lymph node dissection necessary for proximal gastric cancer surgery?

Takahiro Kinoshita et al.

Summary: In Japan, total gastrectomy combined with splenectomy was historically a standard procedure for advanced proximal gastric cancer with metastasis to the splenic hilar lymph nodes. However, due to the disadvantages associated with splenectomy, it is no longer routinely performed. Studies have shown therapeutic value in No. 10 LN dissection for tumors invading the greater curvature, leading to the current recommendation for such patients, although robust evidence is lacking.
