Impaired Bone Fracture Healing in Type 2 Diabetes Is Caused by Defective Functions of Skeletal Progenitor Cells

Summary: This study investigated the mechanisms of impaired fracture healing associated with obesity and type 2 diabetes (T2D). The findings showed that both hyperinsulinemia and insulinopenia delayed bone healing. Hyperinsulinemia resulted in reduced newly formed bone, increased accumulation of marrow adipocytes and senescent cells, while insulinopenia had a more pronounced effect on bone healing. Additionally, serum from obese and T2D subjects inhibited osteoblast differentiation and enhanced adipocyte differentiation in human bone marrow stromal cells.

Bone fragility in diabetes: novel concepts and clinical implications

Summary: Increased fracture risk is a serious complication of diabetes, leading to prolonged immobility and hospitalizations that can result in significant morbidity and mortality. In type 1 diabetes, bone mass and strength are reduced, leading to a five-fold increased risk of lifelong fractures. In type 2 diabetes, fracture risk is increased despite normal bone mass. The fragility of bones in diabetes can be attributed to cellular abnormalities, matrix interactions, immune and vascular changes, and musculoskeletal maladaptation to chronic hyperglycemia.

This study found that individuals with type 2 diabetes had higher cortical porosity and lower cortical thickness in the superolateral femoral neck, and there were significant differences in trabecular microarchitectural parameters. The control group had higher cortical and trabecular bone microhardness. The deteriorated cortical microarchitecture in the superolateral femoral neck cannot be detected by standard DXA measurement.
