Summary: This study investigated the feasibility of using a totally percutaneous approach for in situ microneedle puncture fenestration during thoracic endovascular aortic repair (TEVAR) via access from the left subclavian artery (LSA). The procedure was successfully performed in all 23 patients without any serious complications during the 9-month follow-up period. In situ microneedle puncture fenestration via the LSA is a feasible and effective method for LSA reconstruction during TEVAR.

Marfan syndrome, stent coverage to the level of diaphragm, and the level of celiac artery were independent predictors of DASE. Patients with extended stent coverage to the level of celiac artery have shown a lower incidence of DASE.

Impact of high-risk features and timing of repair for acute type B aortic dissections

Summary: This study aimed to identify the optimal timing of thoracic endovascular aortic repair (TEVAR) for high-risk features (HRF) in type B aortic dissection (TBAD) and investigate the associated outcomes. The results showed that delaying TEVAR for at least 15 days from symptom onset was associated with improved survival, supporting the theory of delaying TEVAR until the subacute phase. Additionally, delaying TEVAR for at least 3 days was associated with a decrease in stroke risk. Having more than one HRF was not statistically associated with worse outcomes.

Systematic review and meta-analysis of current evidence in endograft therapy vs medical treatment for uncomplicated type B aortic dissection

Summary: Although best medical therapy (BMT) is recommended for the treatment of uncomplicated Stanford type B aortic dissection (uSTBAD), there is controversy surrounding thoracic aortic endovascular repair (TEVAR) for uSTBAD. This meta-analysis found that TEVAR was associated with a lower incidence of long-term adverse events, including aortic rupture, reintervention, aorta-related death, and all-cause death, compared to BMT. Additionally, TEVAR significantly contributed to the complete thrombosis of thoracic false lumen and aortic regression.
