Real-life experience with inotersen in hereditary transthyretin amyloidosis with late-onset phenotype: Data from an early-access program in Italy

Marco Luigetti et al.

Summary: This article presents the long-term outcomes of Italian ATTRv patients who received inotersen within an early-access program. The study found that inotersen has a favorable safety profile and that the severity of neurologic disease at baseline is the main factor associated with disease progression.

Summary: The study found that patients with ATTRv-PN and asymptomatic carriers show early nerve pathological and functional changes, with skin biopsy and QST providing valuable insights into disease onset and progression.

Hereditary transthyretin amyloidosis overview

Fiore Manganelli et al.

Summary: Hereditary amyloidogenic transthyretin (ATTRv) amyloidosis is a rare autosomal dominantly inherited disorder caused by mutations in the transthyretin (TTR) gene. The disease is characterized by amyloid deposits and monomer-oligomer toxicity in various systems, with the peripheral nervous system and heart being most affected. Recent advancements in understanding the pathogenesis have led to the development of novel drugs that are rapidly changing the natural history of the disease, posing challenges for timely diagnosis, presymptomatic testing, and management of carriers.

Characteristics of Patients with Late- vs. Early-Onset Val30Met Transthyretin Amyloidosis from the Transthyretin Amyloidosis Outcomes Survey (THAOS)

Marcia Waddington-Cruz et al.

Summary: In the THAOS survey, late-onset ATTRv Val30Met amyloidosis is common and presents with more severe neurological and cardiac manifestations. The heterogeneity of the disease may complicate diagnosis, but increased recognition of late-onset ATTRv Val30Met amyloidosis could lead to improved patient outcomes.

Increased peptidergic fibers as a potential cutaneous marker of pain in diabetic small fiber neuropathy

Pall Karlsson et al.

Summary: Painful diabetic polyneuropathy (DPN) patients showed increased density of dermal peptidergic fibers in the skin, which correlated with pain ratings. This finding offers new insight into the pathophysiological mechanisms of pain in diabetes and suggests potential new therapeutic targets.

The neuropathy in hereditary transthyretin amyloidosis: A narrative review

Stefano Tozza et al.

Summary: Hereditary transthyretin amyloidosis (ATTRv) is an adult-onset condition caused by mutation of the TTR gene, leading to extracellular deposition of amyloid fibrils in tissues, especially in the peripheral nervous system and heart. Despite increased awareness among neurologists, misdiagnosis and delayed correct diagnosis are common. Neuropathy in ATTRv involves progressive axonal loss and demyelination, with features varying based on type and stage of the disease.
