Lung Microbiome in Idiopathic Pulmonary Fibrosis and Other Interstitial Lung Diseases

Francesco Amati et al.

Summary: Interstitial lung diseases are a complex group of diseases with unclear causes and mechanisms of progression. Recent research suggests that the lung microbiome may play a role in the development and progression of these diseases. However, the specific mechanisms are not yet understood. This review aims to discuss the role of altered lung microbiome in various interstitial lung diseases. Understanding the interaction between the host and microbiome in the lungs is a priority for research on these diseases.

The composition of the lung microbiome differs between patients with dermatomyositis and rheumatoid arthritis associated with interstitial lung disease

Yueyan Lou et al.

Summary: Dermatomyositis and rheumatoid arthritis are inflammatory diseases that commonly lead to interstitial lung disease (ILD). Analysis of the lung microbiota composition in patients with these conditions revealed differences in diversity compared to healthy controls, with several unique microbial biomarkers distinguishing between the diseases. Predicted functional and metabolic alterations in the lung microbiota between rheumatoid arthritis, dermatomyositis, and healthy controls may lead to the development of biomarkers for identifying high-risk patients.

Inhibition of lung microbiota-derived proapoptotic peptides ameliorates acute exacerbation of pulmonary fibrosis

Corina N. D'Alessandro-Gabazza et al.

Summary: The authors demonstrate that a monoclonal neutralizing antibody against the proapoptotic peptide corisin, derived from the lung microbiota, can improve acute exacerbation of pulmonary fibrosis and the severity of endotoxin-induced acute lung injury in mice. The study provides insights into the processing and release of corisin and shows that an anticorisin monoclonal antibody can ameliorate lung fibrosis and acute lung injury. These findings highlight the role of corisin in the pathogenesis of the diseases and offer a novel approach to treating the incurable disease.

Gut dysbiosis in rheumatic diseases: A systematic review and meta-analysis of 92 observational studies

Yilun Wang et al.

Summary: Dysbiosis in gut microbiota may be associated with the occurrence or development of rheumatic diseases. The review of existing evidence suggests that there is a decrease in alpha-diversity indices in rheumatic diseases, but the specific associations with different rheumatic diseases are inconsistent. Alterations in certain microbial populations in the gut may be related to rheumatic diseases.

Association between systemic lupus erythematosus and disruption of gut microbiota: a meta-analysis

Shate Xiang et al.

Summary: Recent studies have shown that patients with SLE have imbalanced gut microbiota, characterized by decreased beneficial bacteria and increased harmful bacteria. A meta-analysis of 11 case-control studies demonstrated that SLE patients had lower diversity and richness of gut microbiota compared to healthy controls. Additionally, SLE patients had lower abundance of Ruminococcaceae and higher abundance of Enterobacteriaceae and Enterococcaceae. The use of high doses of glucocorticoids and hydroxychloroquine in SLE treatment was also found to have an impact on the gut microbiota.

Interstitial lung diseases

Marlies Wijsenbeek et al.

Summary: There are over 200 recognized interstitial lung diseases, which vary in rarity. Most of these diseases are characterized by inflammation or fibrosis in the interstitial space, leading to impaired gas exchange and resulting in symptoms such as breathlessness, decreased exercise tolerance, and reduced quality of life. The prognosis for different interstitial lung diseases varies considerably, with some conditions showing reversibility or stabilization, while others lead to respiratory failure and death. Recent advances in the field have led to the approval of drugs for the treatment of specific interstitial lung diseases, including systemic sclerosis-associated interstitial lung disease, interstitial lung disease-associated pulmonary hypertension, and progressive pulmonary fibrosis.

Altered Gut Microbiome in Patients With Dermatomyositis

Sangmee Sharon Bae et al.

Summary: This study compared the microbial composition between patients with dermatomyositis (DM) and healthy controls (HCs), and found that patients with DM, particularly those with interstitial lung disease (ILD)-associated myositis-specific autoantibodies (MSAs), have lower microbial diversity and a distinct taxonomic composition. Predictive metagenomics analysis also revealed functional alterations in multiple metabolic pathways, specifically those involved in lipopolysaccharide synthesis and transport.

Disease Features and Gastrointestinal Microbial Composition in Patients with Systemic Sclerosis from Two Independent Cohorts

Kristofer Andreasson et al.

Summary: The study aims to investigate the alterations in gastrointestinal microbial composition in patients with systemic sclerosis (SSc) and its relationship with SSc features. Dysbiosis was observed in patients recently diagnosed with SSc. Disease duration, interstitial lung disease, small intestinal bacterial overgrowth, and immunosuppression use were associated with fecal microbial composition in SSc patients. Abundance of several pathobiont bacteria differed between the cohorts, suggesting the importance of considering environmental factors in future SSc studies.
