The relationship between islet autoantibody status and the genetic risk of type 1 diabetes in adult-onset type 1 diabetes

Summary: By assessing the genetic risk of type 1 diabetes in autoantibody-negative and -positive individuals, the study found that autoantibody-negative adults with clinician-diagnosed type 1 diabetes had a lower genetic risk compared to autoantibody-positive adults but higher risk compared to type 2 diabetes. The presence of non-autoimmune diabetes was identified as the likely explanation for the lower rate of autoantibody positivity in adult-onset type 1 diabetes.

Heterogeneity of Type 1 Diabetes at Diagnosis Supports Existence of Age-Related Endotypes

Summary: Our study analyzed the characteristics of 6,015 children and adolescents diagnosed with type 1 diabetes between 2003 and 2019. The findings suggest that there are different underlying mechanisms in type 1 diabetes based on the age at diagnosis. Children diagnosed at a younger age showed stronger genetic susceptibility, higher frequency of autoantibodies, and a higher prevalence of affected relatives, while those diagnosed at an older age presented with a higher male preponderance and more severe metabolic decompensation.

Rapid increases in observed incidence and prevalence of Type 1 diabetes in children and youth in Mali, 2007-2016

Summary: From 2007 to 2016, a total of 460 cases of T1D in individuals under 25 years old were diagnosed in Mali, with a male-to-female ratio of 1.04:1 and a peak age of onset at 15-16 years. The incidence of T1D increased from 0.12 to 0.74 per 100,000 population per year, with a consistent rise in prevalence as well. The overall mortality rate was 30.0/1000 patient years, indicating a significant increase in diabetes incidence and prevalence in Mali.

The current definition of LADA identifies a group with features intermediate between typical type 1 and type 2 diabetes, leading to difficulties in classification. Improved approaches are needed to increase test specificity and better understand the detailed pathophysiological components of late-onset autoimmune diabetes.
