Summary: The 2019 coronavirus disease (COVID-19) can cause a hypercoagulable state, leading to various clinical manifestations including arterial and venous thrombosis. Portal vein thrombosis (PVT) is a rare complication rarely reported in COVID-19 patients. In this case, a 49-year-old man with intermittent fevers and generalized weakness for a month was diagnosed with COVID-19 infection, PVT, and pyogenic liver abscess with associated Bacteroides fragilis bacteremia. The patient was treated with antibiotics and anticoagulation, and follow-up was planned after 3 months.
