What Does Disease Progression Look Like in Ulcerative Colitis, and How Might It Be Prevented?

Noa Krugliak Cleveland et al.

Summary: Ulcerative colitis (UC) is characterized by inflammation limited to the mucosa, but persistent clinical disease activity can occur even when mucosal healing has been achieved. UC increases the risk of disease progression and can lead to various complications. Unlike Crohn's disease, halting the progression of UC has been overlooked. This review highlights the progressive nature of UC and proposes further study of its natural history and treatment goals.

Combined Histo-endoscopic Remission but not Endoscopic Healing Alone in Ulcerative Colitis is Associated with a Mucosal Transcriptional Profile Resembling Healthy Mucosa

Cristian Hernandez-Rocha et al.

Summary: This study aimed to establish the prognosis, transcriptional and microbial features of histo-endoscopic remission and activity in ulcerative colitis (UC) patients. The results showed that UC patients with histo-endoscopic remission had a lower risk of relapse and their rectosigmoid mucosa transcriptional profile resembled that of healthy controls. Host genes with antimicrobial properties were significantly associated with mucosal microbiota.

The Role of Innate and Adaptive Immune Cells in the Pathogenesis and Development of the Inflammatory Response in Ulcerative Colitis

Aleksandra Kaluzna et al.

Summary: This review paper summarizes the current understanding of the involvement of the innate and adaptive immune systems in the pathogenesis of ulcerative colitis (UC). It discusses the role of various immune cells, such as neutrophils, dendritic cells, macrophages, innate lymphoid cells, cytotoxic lymphocytes, regulatory T cells, and helper T cells in the development of UC. The review also highlights the influence of B lymphocytes and cytokines on the disease. The paper concludes by mentioning the advancement in immune-targeted treatments for UC, known as biologic agents.

Summary: Mucosal healing is a new target for the treatment of ulcerative colitis, and MES 0 is more strongly associated with a lower rate of clinical relapse compared to MES 1. MES 0 should be considered as the therapeutic target for patients with ulcerative colitis.

Summary: The main goals of ulcerative colitis (UC) treatment are to induce and maintain disease remission, reduce complications, and improve quality of life. Although a cure has not been found, new therapeutic strategies are emerging, while non-biological drugs remain a mainstay of treatment.

Summary: This study aimed to evaluate the impact of disease clearance on long-term outcomes in patients with ulcerative colitis (UC). The results showed that patients with disease clearance had significantly lower risks of hospitalization and surgery. Disease clearance should be considered as a new composite outcome.

Transcriptional Signatures That Define Ulcerative Colitis in Remission

Christopher G. Fenton et al.

Summary: This study used RNA-Seq technology to compare remission, active UC, and healthy control patients, identifying specific transcriptional signatures for UC in remission with changes in gene expression levels. The results showed partial restoration and improvement of homeostasis in the epithelial mucus layer and revival of immunological functions in UC remission patients. The composition of bacterial gut flora may play a key role in UC remission.

Risk of Relapse in Patients With Ulcerative Colitis With Persistent Endoscopic Healing: A Durable Treatment Endpoint

Sushrut Jangi et al.

Summary: Patients with active UC treated-to-target of clinical remission, who achieve and maintain symptomatic remission and EH over consecutive endoscopies, have a low risk of relapse, particularly in a subset of patients who simultaneously achieve histological remission. Therefore, persistent EH may be an important indicator of deep remission.

Increased mucosal IL-12 expression is associated with relapse of ulcerative colitis

Kazuhiko Uchiyama et al.

Summary: Rectal IL-12 and IL-23 expression was elevated in the relapse group, but IL-12 was more strongly associated with UC relapse, irrespective of endoscopic severity and histological activity. Mucosal IL-12 was elevated in patients with deep mucosal healing, suggesting an important role of IL-12 in UC pathogenesis and the molecular mechanism of UC relapse.

Transcriptional Signatures That Define Ulcerative Colitis in Remission
