Impact of Aortic Atresia After Fontan Operation in Patients With Hypoplastic Left Heart Syndrome

Summary: This study aimed to determine the association between aortic atresia and atrioventricular valve function and clinical outcomes after Fontan operation. The results showed that there was no significant difference in transplant-free survival after Fontan operation between patients with aortic atresia and patients with aortic stenosis. However, patients with aortic atresia had a much higher burden of atrioventricular valve failure.

Summary: The study aimed to generate a contractile subpulmonary neo-ventricle from engineered heart tissue (EHT) to actively drive pulmonary flow. By embedding human-induced pluripotent stem cell-derived cardiomyocytes in a fibrin-based hydrogel around a silicone tube, a three-dimensional tubular EHT was created and cultured under continuous flow. The results showed that the EHT developed a synchronous beating pattern and demonstrated coherent pulsatile wave formation with average frequency and pressure.

Fontan failure: phenotypes, evaluation, management, and future directions

Summary: The management of failing and failed Fontan circulation is currently unclear, especially in terms of indications, timing, and type of re-intervention. Factors contributing to complications and mortality differ between children and adults. This study reviewed the literature on failing and failed Fontan circulation, summarizing the clinical phenotypes, diagnostic methods, medical and surgical techniques used, as well as the outcomes. Both surgical and non-surgical options have been explored, including Fontan revision, Fontan conversion, and cardiac transplantation. The results show improvements in peri-operative mortality, but long-term organ dysfunction remains a challenge. Further research is needed to develop more effective strategies for managing failing Fontan circulation.

This review describes different in vitro cardiac models and analyzes their advantages and disadvantages to help researchers choose the most suitable model for their studies. Special attention is given to hiPSC-CMs and three-dimensional (3D) systems, as they can mimic the cytoarchitecture and reproduce the morphological, biochemical, and mechanical features of the human heart.
