Proton Radiation Therapy for Nasopharyngeal Cancer Patients: Dosimetric and NTCP Evaluation Supporting Clinical Decision

Summary: The study proposed an integrated strategy to support the clinical allocation of nasopharyngeal patients between proton and photon radiotherapy. Intensity-modulated proton therapy (IMPT) plans were optimized for 50 consecutive nasopharyngeal carcinoma (NPC) patients treated with volumetric modulated arc therapy (VMAT), and the differences in dose and normal tissue complication probability (Delta NTCPx-p) for 16 models were calculated. The model-based selection (MBS) strategy was used to assess patient eligibility for IMPT based on the results for the most clinically relevant endpoints, applying specific Delta NTCPx-p thresholds and a composite threshold. In addition, a comprehensive toxicity score (CTS) was defined as the weighted sum of all 16 Delta NTCPx-p, where weights follow a clinical rationale. The MBS strategy successfully identifies NPC patients most likely to benefit from IMPT and the CTS summarizes the expected global gain.
