Anatomical distribution of sentinel lymph nodes in patients with endometrial cancer: a multicenter study

Stefano Restaino et al.

Summary: This study aimed to evaluate the anatomical distribution of sentinel lymph nodes (SLNs) and the most frequent locations of nodal metastasis in endometrial cancer patients. The majority of SLNs and positive SLNs were found at the external iliac and obturator level. Older age, higher body mass index, and non-endometrioid histology negatively impacted SLN mapping.

Sentinel lymph node biopsy in high-grade endometrial cancer: a systematic review and meta-analysis of performance characteristics

Summary: Sentinel lymph node biopsy shows good detection rates and diagnostic accuracy in patients with high-grade endometrial cancer, suggesting it can replace complete lymphadenectomies as the standard of care for surgical staging.

ESGO/ESTRO/ESP guidelines for the management of patients with endometrial carcinoma

Summary: The European consensus conference on endometrial carcinoma in 2014 aimed to produce multi-disciplinary evidence-based guidelines. Since then, the European Society of Gynaecological Oncology (ESGO), the European SocieTy for Radiotherapy and Oncology (ESTRO), and the European Society of Pathology (ESP) have decided to update the guidelines and cover new topics to improve care for women with endometrial carcinoma globally.

A phase III randomized clinical trial comparing sentinel node biopsy with no retroperitoneal node dissection in apparent early-stage endometrial cancer - ENDO-3: ANZGOG trial 1911/2020

Andreas Obermair et al.

Summary: The ENDO-3 trial aims to evaluate the value of sentinel node biopsy in the surgical management of endometrial cancer, comparing its effectiveness and potential harms with no retroperitoneal node dissection. The trial design is a phase III, open-label, two-arm, multistage, randomized non-inferiority trial with the primary endpoint being surgical recovery and disease-free survival at 4.5 years.

A cost-effectiveness analysis of three approaches for lymph node assessment in patients with low- and intermediate-risk endometrial cancer

Lara C. Burg et al.

Summary: The study compared the cost-effectiveness of sentinel lymph node mapping, risk factor assessment, and full lymph node dissection in patients with endometrial cancer. Results showed that sentinel lymph node mapping was the most effective strategy for lymph node assessment in low and intermediate-risk patients. Sensitivity analyses demonstrated the robustness of the model to changes in input values.

Development of a surgical competency assessment tool for sentinel lymph node dissection by minimally invasive surgery for endometrial cancer

Kristen Moloney et al.

Summary: Specific mandatory and prohibited steps of sentinel lymph node dissection in endometrial cancer have been identified and validated based on consensus among a large number of international experts. A competency assessment tool is now available and can be used for surgeon selection in clinical trials and for ongoing, prospective quality assurance in routine clinical care. The tool demonstrated high internal consistency and was effective in differentiating between skill levels in surgical videos.

Sentinel lymph node for endometrial cancer treatment: review of the literature

Pier C. Zorzato et al.

Summary: Sentinel lymph node (SLN) mapping has been shown to be a feasible and accurate technique for nodal evaluation in apparently uterine confined endometrial cancer patients, with high-quality evidence supporting this. Additionally, SLN mapping is associated with fewer major postoperative and long-term complications compared to lymphadenectomy (LND). However, there is currently a lack of high-quality evidence on the long-term oncologic safety of this technique compared to standard LND.

Paucity of data evaluating patient centred outcomes following sentinel lymph node dissection in endometrial cancer: A systematic review

Helena M. Obermair et al.

Summary: The study showed that while SLND has advantages in terms of shorter operating times and lower estimated blood loss, as well as potentially reducing the prevalence rates of lymphedema, there is still a lack of conclusive evidence regarding its impact on intra-operative and post-operative complications, adjuvant treatment usage, and overall costs for endometrial cancer patients. The overall quality of evidence on patient-centred outcomes associated with SLND remains poor and susceptible to bias and confounding.
