The use of near-infrared autofluorescence during total laryngectomy with hemi- or total thyroidectomy

Diego Barbieri et al.

Summary: The use of near-infrared autofluorescence in pharyngo-laryngectomy surgery can aid in identifying and preserving parathyroid glands, leading to a lower incidence of postoperative hypocalcemia.

Comparison of Parathyroid Autofluorescence Signals in Different Types of Hyperparathyroidism

Eren Berber et al.

Summary: This study compared the autofluorescence (AF) signals of parathyroid glands across different etiologies of hyperparathyroidism and found that the AF signal patterns of abnormal parathyroid glands were similar across different types of primary hyperparathyroidism. However, parathyroid glands in tertiary hyperparathyroidism exhibited a similar intensity of AF but were more homogeneous compared to those in sporadic primary hyperparathyroidism.

Thyroid gland invasion in total laryngectomy: A systematic review and meta-analysis

Jiaxuan Xie et al.

Summary: This study aimed to identify the predictors and incidence of thyroid invasion in patients with laryngeal cancer. The results showed that subglottic tumors, T4 stage tumors, subglottic extension, and thyroid cartilage invasion were risk factors for thyroid invasion. The overall incidence of thyroid gland invasion was low, indicating that thyroidectomy should be performed based on risk assessment.

Thyroidectomy with and without laryngectomy: Differences in post-operative hypocalcemia and management

Neelam P. Phalke et al.

Summary: This study aimed to compare the impact of total laryngectomy (TL) combined with thyroidectomy on parathyroid function and rates of post-operative hypocalcemia (POH). The results showed a significantly increased risk of hypocalcemia and hypoparathyroidism in TL patients who underwent thyroidectomy, and they also required long-term calcium supplementation.

Risk of Permanent Hypoparathyroidism After Total Thyroidectomy for Benign Disease A Nationwide Population-based Cohort Study From Sweden

Matilda Anneback et al.

Summary: A Swedish study from 2005 to 2015 investigated the prevalence of permanent hypoparathyroidism after total thyroidectomy for benign disease, finding a rate of 12.5%. Risk factors included parathyroid autotransplantation, higher age, female sex, and surgery at a low volume center, with follow-up data possibly underestimating the rate of permanent hypoparathyroidism.

Systematic review of incidence, risk factors, prevention and treatment of post-laryngectomy hypoparathyroidism

Ovie Edafe et al.

Summary: Post-laryngectomy hypoparathyroidism is common and can occur in patients undergoing concurrent thyroidectomy. Additional risk factors include bilateral selective lateral neck dissection, salvage laryngectomy, and total pharyngectomy. Research on prevention and management is lacking in this area and needs to be prioritized.

Hypocalcaemia in pharyngolaryngectomy: Preservation or autotransplantation of parathyroid glands

James D. Every et al.

Summary: Transient and permanent hypocalcaemia rates following partial or total pharyngolaryngectomy were 13% and 10% respectively. Majority of patients underwent salvage surgery for oncological resection, often following radiotherapy. Preservation or autotransplantation of parathyroid glands was shown to reduce permanent postoperative hypocalcaemia.

Current state of intraoperative use of near infrared fluorescence for parathyroid identification and preservation

Carmen C. Solorzano et al.

Summary: Near-infrared fluorescence detection has been widely recognized for identifying or preserving parathyroid glands during thyroid and parathyroid operations, with two main approaches: using near infrared auto-fluorescence to locate parathyroid glands, and using contrast-enhanced near-infrared fluorescence to evaluate gland perfusion.

Fluorescence Image-Guided Surgery for Thyroid Cancer: Utility for Preventing Hypoparathyroidism

Marco Stefano Demarchi et al.

Summary: The most common post-operative complication in thyroid surgery is hypoparathyroidism, especially in cases of thyroid cancer requiring lymph node dissection. Fluorescence imaging is a valuable tool to preserve parathyroid glands and prevent hypoparathyroidism, particularly in cases involving lymphadenectomy.
