Significance of HLA-DQ in kidney transplantation: time to reevaluate human leukocyte antigen-matching priorities to improve transplant outcomes? An expert review and recommendations

Anat R. Tambur et al.

Summary: The importance of HLA matching in kidney allocation algorithms, especially in the US, has decreased due to the introduction of modern immunosuppression, aiming to reduce ethnic/racial disparities in access to transplantation. However, chronic antibody-mediated rejection, primarily caused by de novo antibodies against mismatched donor HLA, remains a leading cause of graft loss, affecting the availability of second transplants for sensitized patients. Recommendations include examining the impact of donor-recipient HLA class II and reconsidering best practices to reduce inequalities while optimizing patient outcomes.

Inflammation Determines the Capacity of Allogenic Endothelial Cells to Regulate Human Treg Expansion

Amy Rachael Cross et al.

Summary: This study investigates the impact of inflammation on endothelial regulation of CD4(+)Treg cells. The findings suggest that high levels of inflammation can impair endothelial cell function and lead to inhibition of Treg differentiation. Blocking PD-L1 can partially restore Treg cell numbers.

Hidden Patterns of Anti-HLA Class I Alloreactivity Revealed Through Machine Learning

Angeliki G. Vittoraki et al.

Summary: This study analyzed anti-HLA class I responses in a cohort of 1,066 patients from one country using unsupervised machine learning approaches to study clustering patterns and cross-reactive patterns of alloreactivity. The research revealed distinct projections of anti-HLA-A and anti-HLA-C responses, identified new cross-reactive groups, and described major population-specific cross-reactive groups based on amino acid combinations. The interpretation of the results was supported by current knowledge of antigenic targets characterized experimentally or computationally.
