Continued potassium supplementation use following loop diuretic discontinuation in older adults: An evaluation of a prescribing cascade relic

Grace Hsin-Min Wang et al.

Summary: Based on the analysis of Medicare data, we found that it is common for older adults to receive potassium supplementation simultaneously or after loop diuretic initiation, and some patients continue to use potassium even after discontinuing the loop diuretic. This phenomenon, known as relics, highlights the need for clinicians to be aware of and address unnecessary drug use.

Improving the specificity of drug-drug interaction alerts: Can it be done?

Thomas Reese et al.

Summary: Inaccurate and nonspecific medication alerts can lead to high override rates, alert fatigue, and patient harm. This study highlights the inconsistency of drug value sets that trigger DDI alerts and demonstrates a method of classifying risk-modifying factors to improve alert specificity.

Use of serotonin reuptake inhibitor antidepressants and the risk of bleeding complications in patients on anticoagulant or antiplatelet agents: a systematic review and meta-analysis

Surapon Nochaiwong et al.

Summary: This systematic review and meta-analysis found that the use of serotonin reuptake inhibitor (SRI) antidepressants in patients receiving antithrombotic therapy is associated with a higher risk of bleeding complications, including major bleeding, gastrointestinal bleeding, and any bleeding events. The risk is elevated among individuals receiving anticoagulants or antiplatelet therapy. Caution should be exercised in co-prescribing SRIs with antithrombotic therapy due to safety concerns regarding bleeding risks.

Contextualized Drug-Drug Interaction Management Improves Clinical Utility Compared With Basic Drug-Drug Interaction Management in Hospitalized Patients

Arthur T. M. Wasylewicz et al.

Summary: This study compares the clinical utility of contextualized and basic DDI management in hospitalized patients. The results show that contextualized DDI management has higher positive predictive value and number of pharmacy interventions, and no DDIs were missed, indicating that it is able to better capture relevant alerts while reducing irrelevant DDI alerts.

Drug interaction alerts in older primary care patients, and related medically justified actions

Carina Tukukino et al.

Summary: This study described the interaction alerts in older patients and evaluated whether these alerts required further medical action or had already been addressed. The results showed that most alerts had already been addressed or were not relevant in the clinical setting, but some patients still needed medical action.

Opioid Drug-Drug-Drug Interactions and Unintentional Traumatic Injury: Screening to Detect Three-Way Drug Interaction Signals

Emily K. Acton et al.

Summary: Growing evidence suggests that drug interactions may be responsible for the association between opioid use and unintentional traumatic injury. Using high-throughput screening of US commercial health insurance data, signals of opioid interactions with commonly co-dispensed medications leading to unintentional traumatic injury were identified.

Adverse drug reactions, multimorbidity and polypharmacy: a prospective analysis of 1 month of medical admissions

Rostam Osanlou et al.

Summary: This study examines the burden and cost of adverse drug reactions (ADRs), polypharmacy, and multimorbidity. The researchers find that ADRs contribute to a significant number of hospital admissions, with polypharmacy and multimorbidity being important factors. They suggest that reducing inappropriate polypharmacy should be prioritized to prevent ADRs.

Drug-Related Hospital Admissions via the Department of Emergency Medicine: A Cross-Sectional Study From the Czech Republic

Zuzana Ocovska et al.

Summary: This study investigated the prevalence, preventability, and medication classes involved in drug-related hospital admissions (DRAs). The findings showed that DRAs are a significant cause of hospital admissions, with both prescribing and patient-related issues contributing to preventability. The most common medication classes associated with DRAs included antithrombotic agents, diuretics, and drugs used in diabetes. It is essential to balance the benefits and risks of medications in managing DRAs.

Hannah De Schutter et al.

Summary: The purpose of this study was to update and validate the cardiovascular segment of the RASP list, a screening tool for optimizing geriatric polypharmacy. The study involved three steps, including updating the tool based on literature and expert input, internal discussions with cardiologists and pharmacists, and a Delphi consensus method with the input of various healthcare professionals. The final construct of the RASP_CARDIO list includes 95 statements related to potentially inappropriate prescribing of cardiovascular agents.

Identifying and Characterizing Serious Adverse Drug Reactions Associated With Drug-Drug Interactions in a Spontaneous Reporting Database

Lara Magro et al.

Summary: The study found that about one-third of patients exposed to potential drug-drug interactions actually experienced serious adverse drug reactions, and that a spontaneous reporting database can be used to identify and describe ADRs caused by drug-drug interactions. Among these ADRs, warfarin was the most frequently reported interacting drug and the most common ADRs were gastrointestinal or cerebral hemorrhagic events.

An International Consensus List of Potentially Clinically Significant Drug-Drug Interactions in Older People

Pauline Anrys et al.

Summary: This study aimed to establish an explicit list of potentially clinically significant drug-drug interactions (DDIs) in individuals aged over 65 years. Through a multidisciplinary expert panel and a Delphi survey, consensus was achieved on 66 potentially clinically significant DDIs, most of which involved cardiovascular, antithrombotic, and central nervous system drugs. The final list includes information on the mechanism of interaction, harm, and management, with treatment modification recommended for three-quarters of DDIs.

Eric Chou et al.

Summary: The study aims to develop, validate, and test DDI alert algorithms that utilize patient context available in electronic health records data. Synthetic health records data was used for validation and real-world hospital data was used for testing, showing the potential of the algorithms to significantly reduce alert interruptions in clinical workflow. The 8 DDI algorithms developed in the study are a step towards addressing the critical need for DDI alerts that are more specific to patient context than current commercial alerting systems.
