The role of environmental exposures and gene-environment interactions in the etiology of systemic lupus erythematous

Summary: Systemic lupus erythematosus (SLE) is a complex chronic autoimmune disease caused by both genetic and environmental factors. Genetic risk factors account for only one-third of the observed heritability, with the remaining risk attributed to environmental exposures and gene-environment interactions. Occupational exposure to crystalline silica, current smoking, and exogenous estrogens are positively associated with SLE risk. Other factors like lifestyle behaviors, UV radiation, air pollution, solvents, pesticides, vaccines and medications, and infections may also contribute to SLE risk. Alcohol use may be associated with decreased risk. Gene-environment interactions involving IL-10, ESR1, IL-33, ITGAM, and NAT2, along with smoking, UV exposure, and alcohol, have been found to influence SLE risk. Understanding these risk factors and their interactions can help in understanding SLE pathogenesis and developing preventive interventions.

Peripheral immunophenotypes associated with the flare in the systemic lupus erythematosus patients with low disease activity state

Summary: Systemic lupus erythematosus (SLE) is an autoimmune disease characterized by clinical heterogeneity and flare unpredictability. The association between SLE flare and immunophenotypes was explored in this study. Flow cytometric analysis revealed abnormal proportions of B and T cell subsets in patients with low disease activity state (LDAS) compared to healthy controls. Principal component analysis (PCA) and cluster analysis (CA) helped differentiate the immunophenotypes. Patients were divided into 3 groups, and it was found that the group with lower proportions of memory B cells had a lower risk of flare compared to the group with higher proportions of naive B cells and T cells.

High-intensity immunosuppressive therapy followed by less intensive therapy is typically used to control disease activity in severe cases of SLE.
