Severe acute respiratory syndrome coronavirus-2-associated cholangiopathies

Alessandra Bartoli et al.

Summary: Two different cholangiopathies have been reported in patients after severe-COVID-19, which are associated with the autoimmune response triggered by the viral infection. One is primary biliary cholangitis (PBC) developed after the infection, and the other is a rapidly progressing cholangiopathy with features of secondary sclerosing cholangitis (SSC). Attention should be paid to the development of cholestasis in ICU setting and after discharge, and periodic liver function tests should be performed. Currently, there are no treatment strategies available, and liver transplantation is the last option for individuals with liver failure due to SSC.

Summary: Drug-induced liver injury (DILI) can cause a range of clinical and biochemical manifestations, and may result in chronic injury and various long-term sequelae, including vanishing bile duct syndrome, fibrosis, and autoimmune hepatitis. However, little is known about the psychological consequences of DILI, highlighting the need for further research.

Summary: IgG4-related sclerosing cholangitis (IgG4-SC) can be diagnosed based on cholangiographic classification and clinical diagnostic criteria, with a close association with autoimmune pancreatitis (AIP). In Japan, the 2020 clinical diagnostic criteria have been established, including narrowing of the intrahepatic and/or extrahepatic bile duct, thickening of the bile duct wall, serological and pathological findings, among others.

Serological biomarkers for management of primary sclerosing cholangitis

Summary: The clinical manifestations and progression of primary sclerosing cholangitis (PSC) vary, and its pathogenesis is not well understood. Gut-liver interactions play an important role in the development of PSC. Recent advances in biomarkers research may help identify clinically relevant PSC subgroups and assist in diagnosis, disease stratification, and potential therapeutic targets.

The Risk Assessment and Clinical Research of Bile Duct Injury After Transcatheter Arterial Chemoembolization for Hepatocellular Carcinoma

Summary: Bile duct injury after TACE for hepatocellular carcinoma is not caused by a single factor but by a variety of factors, closely related to microvascular compromise and chronic biliary infection. Lipiodol deposition along the bile duct wall and a sharp increase in ALP > 200 U/L within a week after TACE can predict bile duct injury, with patients who have undergone prior hepatectomy and proximal arterial chemoembolization having a significantly increased probability of bile duct injury.

Summary: Primary sclerosing cholangitis (PSC) is a rare chronic liver disease characterized by bile duct inflammation, fibrosis, and cirrhosis, often associated with inflammatory bowel disease (IBD) and diagnosed in adulthood. Currently, there is no effective medical therapy for PSC, and management focuses on symptom alleviation and complication prevention. Liver transplantation (LT) remains the only life-extending treatment for end-stage PSC, with a high rate of recurrence and post-transplant morbidity.
