Summary: This article summarizes the results of six trials that assessed the efficacy of mechanical thrombectomy (MT) alone versus intravenous thrombolysis (IVT) plus MT for anterior circulation large vessel occlusion (LVO) ischemic stroke. The findings suggest that for patients within 4.5 hours of symptom onset, receiving IVT plus MT directly is more effective than receiving MT alone. For patients admitted to centers without MT facilities, receiving IVT first and then being rapidly transferred to MT capable-centers is a better option.
