Effect of angiotensin receptor neprilysin inhibitors on left atrial remodeling and prognosis in heart failure

Yuxi Sun et al.

Summary: This study found that compared to ACEI/ARB, ARNI has greater effects on left atrial reverse remodeling and is associated with a better prognosis in heart failure patients. For patients with mildly abnormal left atrial volume index, ARNI use can significantly reduce mortality.

Summary: The 2022 AHA/ACC/HFSA Guideline for the Management of Heart Failure provides patient-centric recommendations for clinicians to prevent, diagnose, and manage patients with heart failure. It is based on contemporary evidence and aims to improve quality of care and align with patients' interests.

Association of Left Atrial Structure and Function With Heart Failure in Older Adults

Riccardo M. Inciardi et al.

Summary: This study assessed the left atrial (LA) structure and function in older adults without prevalent heart failure (HF) and found that LA measures were associated with NT-proBNP and increased risk of incident HF or death. These measures, when added to traditional risk factors and NT-proBNP, improved the accuracy of predicting cardiovascular events.

Left atrial reverse remodeling improves risk stratification in patients with heart failure with recovered ejection fraction

Masayuki Shiba et al.

Summary: This study aimed to investigate the relationship between left atrial reverse remodeling and prognosis of heart failure with recovered ejection fraction. The results showed that in the heart failure group with recovered ejection fraction, left atrial reverse remodeling was associated with a lower incidence of the primary outcome measure, while no association was found in the group without recovered ejection fraction. The combination of left ventricular and atrial reverse remodeling may help in improving risk stratification for heart failure.

Cardiac Structure and Function and Diabetes-Related Risk of Death or Heart Failure in Older Adults

Riccardo M. Inciardi et al.

Summary: This study aimed to investigate whether cardiac structure and function abnormalities associated with dysglycemia could explain the increased risk of death or heart failure. The findings showed that cardiac abnormalities partially explained the increased risk of events in patients with diabetes.

Characteristics and outcomes of heart failure with recovered left ventricular ejection fraction

Xinxin Zhang et al.

Summary: There is a growing interest in understanding the natural history and prognosis of heart failure patients with recovered ejection fraction compared to persistent HFrEF. HFrecEF is a distinct phenotype with better clinical outcomes, particularly in terms of lower short-term mortality rates.

Heart Failure With Midrange Ejection Fraction: Prior Left Ventricular Ejection Fraction and Prognosis

Xinxin Zhang et al.

Summary: This study found that HFmrEF patients are heterogeneous with three different subsets identified, each with different outcomes. The association between change in LVEF and adverse outcomes remained significant even after fully adjusting for all-cause mortality and composite outcome. Strategies for managing HFmrEF should include previously measured LVEF to allow stratification based on direction changes in LVEF for better treatment optimization.

Summary: This study showed that carriers of the V122I TTR variant in African-Americans have significant left atrial enlargement and dysfunction compared to non-carriers, suggesting that abnormalities in left atrial function may be early markers of subclinical disease in this population.

Summary: This study found that heart failure with recovered ejection fraction is a distinct phenotype with better clinical outcomes compared to persistent HFrEF. HFrecEF patients have relatively better short-term survival at 24 months, but no significant difference thereafter.
