Solving the Pulmonary Hypertension Paradox in Patients With Severe Tricuspid Regurgitation by Employing Artificial Intelligence

Vera Fortmeier et al.

Summary: This study aimed to improve the assessment of pulmonary hypertension (PH) in patients with severe tricuspid regurgitation (TR) using echocardiography. An extreme gradient boosting (XGB) algorithm was trained to predict mean pulmonary artery pressure (mPAP) based on echocardiographic parameters. The results showed that echocardiography consistently underestimated systolic pulmonary artery pressure compared to right heart catheterization. The XGB algorithm using 9 echocardiographic parameters could reliably predict mPAP levels. Patients with elevated predicted mPAP levels showed significantly reduced 2-year survival after transcatheter tricuspid valve intervention (TTVI). This study highlights the importance of accurate assessment of PH in patients with severe TR.

A Score to Assess Mortality After Percutaneous Mitral Valve Repair

Sergio Raposeiras-Roubin et al.

Summary: This study aimed to create and validate a user-friendly score (MitraScore) to predict the risk of mortality in patients undergoing transcatheter edge-to-edge mitral valve repair (TEER). The MitraScore, derived from 8 independent predictors, showed better discrimination and calibration in predicting mortality compared to existing scores. It also demonstrated predictive value for heart failure rehospitalization and correlated with the probability of clinical improvement.

Evolution and Prognostic Impact of Cardiac Damage After Aortic Valve Replacement

Summary: This study found that the extent of extravalvular cardiac damage at baseline and its change at 1 year after AVR have important prognostic implications for patients. Monitoring cardiac damage can help predict patients' survival rates and rates of heart failure hospitalization.

Transcatheter Mitral Valve Repair in Patients With Atrial Functional Mitral Regurgitation

Philipp Doldi et al.

Summary: This study analyzed the outcome of patients with atrial functional mitral regurgitation (aFMR) undergoing mitral valve transcatheter edge-to-edge repair (M-TEER). The results showed that M-TEER effectively improved symptoms and resulted in a high 2-year survival rate for aFMR patients. Advanced heart failure symptoms and right ventricular dysfunction were identified as important risk factors for the prognosis of aFMR patients.

Summary: This study categorized patients with severe aortic stenosis (AS) into 4 distinct phenotypes using unsupervised machine learning. Structural alterations in left and right heart morphology may be equally sensitive indicators of poor prognosis compared with high-grade pulmonary hypertension.

Impact of Right Ventricular Dysfunction on Outcomes After Transcatheter Edge-to-Edge Repair for Secondary Mitral Regurgitation

Nicole Karam et al.

Summary: The study assessed the impact of right ventricular dysfunction on survival after TMVR for severe SMR and found that it was a major predictor of adverse outcomes. The findings emphasize the importance of assessing right ventricular function when planning TMVR for SMR patients.

Prognostic Implications of Associated Cardiac Abnormalities Detected on Echocardiography in Patients With Moderate Aortic Stenosis

Mohammed Rizwan Amanullah et al.

Summary: This study evaluated the prevalence and prognostic value of the extent of extra-aortic valvular cardiac abnormalities in patients with moderate AS. The results showed a significant association between increasing extent of extra-aortic valvular cardiac abnormalities and higher mortality rates. Particularly, patients with involvement of the left atrial or mitral valve and above were independently associated with all-cause mortality.

Machine Learning Identifies Clinical Parameters to Predict Mortality in Patients Undergoing Transcatheter Mitral Valve Repair

Elric Zweck et al.

Summary: A machine learning-based risk stratification tool was developed for predicting 1-year mortality in TMVR patients, showing higher predictive accuracy in external validation compared to existing clinical scores. The model, named MITRALITY, utilized 6 baseline clinical features for prediction, offering potential benefits for future clinical trials and daily clinical practice.
