Trends in inpatient and post-discharge mortality among young infants admitted to Kilifi County Hospital, Kenya: a retrospective cohort study

Alison Talbert et al.

Summary: The number of young infants admitted to a county hospital in Kenya is increasing. The inpatient case fatality rate is 16% and the 1-year post-discharge mortality rate is 5.7%. Inpatient deaths are associated with disease severity, while post-discharge deaths are associated with hospitalization length, leaving against advice, and referral. This highlights the need for improved post-discharge care and services.

Etiology of severe invasive infections in young infants in rural settings in sub-Saharan Africa

Estomih Mduma et al.

Summary: This study aimed to identify pathogens responsible for serious invasive infections in young infants in sub-Saharan Africa and describe their antibiotics resistance profile. The most frequent pathogens identified were Klebsiella pneumonia, Staphylococcus aureus, and Escherichia coli. However, most cases remained without a definite diagnosis, highlighting the need for more accurate diagnostic tools.

Clinical features to distinguish meningitis among young infants at a rural Kenyan hospital

Christina W. Obiero et al.

Summary: Detection of meningitis in young infants remains challenging, with WHO-recommended signs showing limited sensitivity and specificity. History of fever improves sensitivity but reduces specificity, highlighting the need for careful evaluation and appropriate diagnostic tools for management.

Impact of free maternity policies in Kenya: an interrupted time-series analysis

Stacey Orangi et al.

Summary: The free maternity policies in Kenya have shown mixed effects in increasing access to maternal health services. While the 2013 policy led to an increase in normal deliveries and caesarean sections in public facilities, the 2017 Linda Mama programme resulted in a decrease then an increase in postnatal care visits and a slight decrease in caesarean sections in public facilities. In private and faith-based facilities, there were different trends observed in the utilization of maternal health services following the policies.

Burden of disease and risk factors for mortality amongst hospitalized newborns in Nigeria and Kenya

Helen M. Nabwera et al.

Summary: This study focuses on describing the patient population, priority diseases, and outcomes of newborns admitted to neonatal units in Nigeria and Kenya. Findings show high proportions of low birthweight and very preterm infants, with common morbidities being jaundice, suspected sepsis, respiratory conditions, and birth asphyxia. Mortality rates were significantly high among very low birthweight and very preterm infants. Factors associated with mortality included gestation period, birthweight, congenital anomalies, and maternal antibiotic use, with interventions like dexamethasone treatment potentially reducing mortality rates. Efforts are needed to address the high burden of illnesses and mortality in hospitalized newborns in sub-Saharan Africa.

A cohort study of survival following discharge from hospital in rural Tanzanian children using linked data of admissions with community-based demographic surveillance

Oscar Mukasa et al.

Summary: This study highlights the importance of linking individual health data from hospitals with community-based data in a poor rural setting. The research found that children who survive hospital discharge have a higher mortality risk compared to those who have not been admitted, especially infants and those admitted for pneumonia and diarrhea. Children living further from the hospital also face higher mortality rates post-discharge, suggesting the need for additional support for families after hospitalization.
