Identification of disease-causing variants by comprehensive genetic testing with exome sequencing in adults with suspicion of hereditary FSGS

Next-generation sequencing in patients with familial FSGS: first report of collagen gene mutations in Tunisian patients

Summary: FSGS is a histological lesion with many causes, presenting with significant proteinuria. In this study, genetic testing using NGS identified mutations in seven Tunisian families, with newly discovered mutations in NPHS2, PLCE1, PAX2, COL4A3, and COL4A4 genes. Therefore, not only podocyte genes but also COL4A3/4 mutations should be considered in patients with FSGS.

Prevalence Estimates of Predicted Pathogenic COL4A3-COL4A5 Variants in a Population Sequencing Database and Their Implications for Alport Syndrome

Summary: This study estimated the frequencies of predicted pathogenic COL4A3-COL4A5 variants in populations without known kidney disease. The COL4A5 variants were found in at least one in 2320 individuals, and the predicted pathogenic heterozygous COL4A3 and COL4A4 variants affected one in 106 of the population. The results suggest the population frequencies for Alport syndrome need to be adjusted for individual variant penetrance and other factors.

Identification of Genetic Causes of Focal Segmental Glomerulosclerosis Increases With Proper Patient Selection

Summary: The study analyzed the clinical and histologic characteristics of patients with FSGS lesions to increase the likelihood of finding a causative genetic variant. Proper patient selection significantly increased the rate of positive genetic testing, with individuals with a family history of kidney disease, absence of nephrotic syndrome, and female sex being strong predictors of finding a causative genetic variant. The majority of individuals with undetermined FSGS had a genetic diagnosis.
