Between-trial heterogeneity in ARDS research

J. Juschten et al.

Summary: Most RCTs in ARDS patients exhibit significant heterogeneity in terms of reporting standards and outcomes, with a wide range of control group mortality rates and inconsistencies in reported baseline patient characteristics. Standardized reporting of trial and patient characteristics is urgently needed to improve the generalizability of ARDS research.

A Prediction Model for Assessing Prognosis in Critically Ill Patients with Sepsis-associated Acute Kidney Injury

Hongbin Hu et al.

Summary: A predictive model for in-hospital mortality of SA-AKI in critically ill patients was developed in this study, which included age, admission type, liver disease, metastatic cancer, lactate, BUN/SCr, admission creatinine, positive culture, and AKI stage as predictors for survival. The model showed good predictive performance in both the training and validation cohorts.

Unsuccessful and Successful Clinical Trials in Acute Respiratory Distress Syndrome: Addressing Physiology-Based Gaps

Jesus Villar et al.

Summary: ARDS is a severe lung condition that can be challenging to treat effectively, with conventional therapies like intensive care and positive end-expiratory pressure often being insufficient for some patients. Mechanical ventilation remains the standard support technique, providing time for the lungs to recover, but it can also have adverse effects due to changes in lung pressures. Various techniques and therapies, such as neuromuscular blocking agents, prone positioning, and high-frequency oscillatory ventilation, are being tested to improve oxygenation and ventilation in ARDS patients. Future clinical trials in ARDS may need to address gaps in current knowledge and explore new treatment approaches.

Stratification for Identification of Prognostic Categories In the Acute RESpiratory Distress Syndrome (SPIRES) Score

Jesus Villar et al.

Summary: The study developed a scoring model for stratifying patients with acute respiratory distress syndrome into risk categories for early prediction of ICU death. The scoring model was validated with clinical data from multiple observational cohorts, showing better prediction of ICU death compared to traditional indicators like Pao(2) to Fio(2) ratio.
