Effect of Lower Tidal Volume Ventilation Facilitated by Extracorporeal Carbon Dioxide Removal vs Standard Care Ventilation on 90-Day Mortality in Patients With Acute Hypoxemic Respiratory Failure The REST Randomized Clinical Trial

James J. McNamee et al.

Summary: In this study, lower tidal volume mechanical ventilation with extracorporeal carbon dioxide removal did not significantly reduce 90-day mortality in patients with acute hypoxemic respiratory failure compared to conventional low tidal volume mechanical ventilation. The study was terminated early and may have been underpowered to detect a clinically important difference.

Personalized mechanical ventilation in acute respiratory distress syndrome

Paolo Pelosi et al.

Summary: A personalized mechanical ventilation approach based on lung physiology, ARDS etiology, lung imaging, and biological phenotypes may improve ventilation practice and outcomes for ARDS patients, but further research is needed before it can be applied clinically. Ventilatory parameters should be titrated based on physiological variables and individualized goals, with a focus on low tidal volume and driving pressures.
