The Trifecta Study: Comparing Plasma Levels of Donor-derived Cell-Free DNA with the Molecular Phenotype of Kidney Transplant Biopsies

Summary: The donor-derived cell-free DNA fraction (dd-cfDNA[%]) in plasma at the time of indication biopsy is strongly correlated with gene expression in the biopsied allograft, particularly in cases of active molecular rejection. This correlation has the potential to reduce unnecessary biopsies in kidney transplant recipients.

Correlation Between Microvascular Inflammation in Endomyocardial Biopsies and Rejection Transcripts, Donor-specific Antibodies, and Graft Dysfunction in Antibody-mediated Rejection

Summary: This study evaluated the biological and clinical value of a semiquantitative assessment of the extent of microvascular inflammation (MVI) in endomyocardial biopsies (EMBs). The results showed that the MVI score was associated with tissue rejection molecular activity, pathology classification, antibody expression, and graft dysfunction. These findings suggest that MVI assessment is important for the diagnosis and treatment of cardiac antibody-mediated rejection.

Correlation of Donor-derived Cell-free DNA With Histology and Molecular Diagnoses of Kidney Transplant Biopsies

Summary: This single-center prospective study examines the application of circulating donor-derived cell-free DNA (cfDNA), molecular microscope diagnostic system (MMDx), and traditional histology in kidney transplant rejection. The results show that MMDx provides higher precision than histology, but there are discrepancies between the two methods. Additionally, cfDNA correlates strongly with molecular acute kidney injury, indicating an association with tissue injury regardless of rejection characteristics.

Factors associated with kidney graft survival in pure antibody-mediated rejection at the time of indication biopsy: Importance of parenchymal injury but not disease activity

Summary: In this study, the focus was on clinical and molecular features of kidney transplant patients after indication biopsy to predict the risk of transplant failure. The results showed that factors such as estimated GFR, proteinuria, time posttransplant, donor-specific antibody, as well as molecular and histologic features reflecting injury were more important predictors of failure compared to rejection activity.

Cell-free DNA beyond a biomarker for rejection: Biological trigger of tissue injury and potential therapeutics

Summary: Cell-free DNA, measured as donor-derived cell-free DNA, is being developed as a non-specific biomarker for allograft injury and transplant rejection. However, the characteristics of cell-free DNA are more specific and can be used to phenotype transplant rejection and point to underlying molecular mechanisms. Novel treatment approaches targeting these cell-free DNA pathways may be useful in treating rejection and preventing organ dysfunction.
