Summary: The migration of leukocytes into the CNS plays a crucial role in the pathogenesis of MS. The monoclonal antibody natalizumab has proven to be an effective treatment for RRMS by obstructing this process. However, it is not able to control PMS and may cause severe side effects. In this study, the researchers identified DICAM as a cell trafficking molecule preferentially expressed by T(H)17-polarized CD4(+) T lymphocytes. Blocking DICAM reduced T(H)17 cell migration across the blood-brain barrier and alleviated disease symptoms in mouse models, suggesting that it could be a promising therapeutic target.

Summary: This study reveals the important role of tissue-resident memory T cells, particularly pathogenic CD8(+) T cells, in chronic autoimmune diseases of the central nervous system. These cells are crucial for sustaining chronic inflammation and progressive loss of neurons, and they exhibit phenotypic and functional heterogeneity. The presence and functional output of tissue-resident CD8(+) T cells depend critically on CD4(+) T cells. Therefore, therapies targeting this compartmentalized autoreactive T cell subset may be effective for treating CNS autoimmune diseases.

The results showed that active and mixed white matter MS lesions with foamy microglia exhibited high acute axonal damage and correlated with elevated CSF NfL levels. These findings support the use of CSF NfL as a biomarker to monitor inflammatory demyelinating lesion activity with axonal damage in MS.

The critical role of complement component 1q (C1q) in the activation of microglia inflamed in MS has been identified and potential therapeutic avenues, such as C1q inhibition, have been suggested for addressing chronic white matter inflammation.
